JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. PAD Case Managers are responsible for assisting individuals to make full use of Behavioral Health Services, other community social services and natural supports which will help individuals achieve a more independent and fulfilling life. Services provided will include developing a partnership with the individual to assess needs, obtain services, resolve crises, and meet the requirements of the PAD agreement. Through this partnership the individual and Case Manager work together to plan, coordinate, monitor and advocate for the services and supports needed to achieve the individual’s goals. Work cooperatively with other departments and agencies to ensure successful client engagement, monitoring and billing. Maintain a positive relationship with the Philadelphia Police Assisted Diversion services officers and staff.
The hours worked will be from 4PM - 12:30AM, Monday - Friday.
QUALIFICATIONS:
- Bachelor’s Degree in Human Services or equivalent work experience and education.
- Human services or criminal justice work experience if degree is not in human services.
- Valid Driver’s License and use of personal vehicle.
- Basic word processing and computer skills.
- Good Communication Skills.
- COVID-19 vaccination or willingness to receive vaccine required.
SPECIFIC DUTIES:
- Provide coverage of PAD program phone.
- Complete initial contact records and acquire all needed releases of information.
- Identify accessibility needs for individuals at the time of initial contact to ensure the needs are met.
- Develop partnerships with assigned individuals to assess needs, obtain services and resolve crises.
- Assist individuals with fulfilling diversion agreement determined by the Philadelphia Police PAD officers.
- Ensure all required documentation is completed in a timely, comprehensive, and legible manner and complies with PATH policies and procedures.
- Link individuals to the most appropriate programs/services to meet their needs such as psychiatric and medical treatment, substance use disorder treatment, residential services, financial entitlements, social/recreational activities, and other general social services.
- Utilize the ASAM criteria assessment tool with individuals who are in need of substance abuse services.
- Participate in crisis assessment to ensure individual receives necessary supportive intervention and assistance in managing and resolving the crisis and develop a crisis plan to assure coordination of recommended disposition.
- Notify supervisory staff of emergencies and urgent issues in a timely manner.
- Complete or assist others in completing an involuntary commitment petition.
- Make approved home visits.
- Monitor those supports and services for appropriateness and effectiveness.
- Other duties as assigned.
