We are looking for an Overnight Case Manager to enhance the quality of resident management overnight, maximize satisfaction and promote site structure. The successful candidate will be accountable for promoting exceptional responses during their shift. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the residents health and human services needed. An experienced Case Manager is a MUST.
Responsibilities
- Maintain alertness during the overnight shift as you’re frequently watching security cameras in our corporate office and providing effective communication with on site staff that is monitoring the youth per agency policies and procedures
- Provide crisis intervention services on the telephone for behavioral children, for the youth in our group home setting, if needed, leave site to transport youth to hospital for psyche evaluations
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and resident-centered
- Provide case management services to residents, by identifying necessary resources and services within the community and setting up partnerships, i.e mentorship programs, jobs, colleges, tutoring, religion etc…
- Afterhour intake services
- Assist residents in crisis management situations, providing immediate support and connecting them with appropriate resources
- Assist 1st shift Case Managers with minor paperwork task that is due urgently and wasn’t able to complete during day shift due to call off (OnCall, or emergency)
- Record case information, accurately complete all necessary forms and produce professional reports
- Organization and collection of records to be filed away and/or uploaded electronically at corporate office
- Transport to and from early morning health and mental health appointments, job interviews, school etc…
- Develop effective working relations and cooperate with the treatment team throughout the entire case management process and facilitate multiple care aspects (case coordination, information sharing, etc)
- Arrange access and execute clear communication internally as well as with social services, health related and other governmental agencies
- Take the extra mile and interact with residents to keep track of their progress and to ensure satisfaction
- Adhere to professional standards as outlined by protocols, rules and regulations
Skills
- Proven working experience in case management, including as a nurse, medical, mental health care manager or a related job
- Excellent knowledge of case management principles.
- Previous experience with psychological aspects of care
- Effective communication skills
- Excellent organizational and time management skills in a MUST
- Excellent written skills
- Familiarity with professional and technical emerging knowledge
- Problem solving skills and ability to multi-task
- Have the ability to work alone or with a team
