We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and managing the care of patients in a hospital setting. Responsibilities:
- Conduct comprehensive patient assessments to determine individual needs and develop appropriate care plans
- Collaborate with healthcare professionals, including physicians, nurses, and social workers, to coordinate patient care
- Monitor patient progress and adjust care plans as necessary
- Advocate for patients and ensure their rights are protected
- Maintain accurate and up-to-date medical records
- Provide education and support to patients and their families
- Collaborate with insurance providers to ensure appropriate coverage for necessary treatments
- Stay current on medical advancements, treatment options, and best practices in case management
Requirements:
- Bachelor's degree in nursing or a related field (Master's degree preferred)
- Valid nursing license (RN) or relevant certification
- Minimum of 3 years of experience in case management or a related role
- Strong knowledge of physiology and medical terminology
- Experience working in an acute care or ICU setting is highly desirable
- Familiarity with managed care principles and practices
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a multidisciplinary team
- Strong organizational skills and attention to detail
