OVERVIEW:
The Case Manager/Mental Health Coordinator will work closely with the Director of Programs and referring agencies to identify, screen and access prospective program participants, including conducting assessments relative to their substance abuse needs, Mental Health concerns and housing support. This position is responsible to provide both case management program services and Mental Health services; such as achieve stability, increased levels of income, and greater self- determination. This position will be knowledgeable with community referral resources and have creative problem-solving skills in a fast-paced environment; and will work in collaboration with the referring agencies and property management to help clients achieve their goal of independent living.
DUTIES AND RESPONSIBILITIES:
- Conduct assessments, provide case management support and other services
- Lead Mental Health and substance use disorder groups and individual cases
- Maintain records of all group meetings, resident participation, and attendance
- Provide statistical data
- Work with referring agencies and community partners with referrals, screening admissions, and integrating treatment plans
- Lead psychosocial/psychoeducational groups such as anger management, stress relief, grief and loss, anxiety, fear, self-esteem, mild depression, and interpersonal relationships and provide other effective coping skills as needed
- Facilitate at least one process group and life skills class once per month
- Provide referrals for clients who need medical or more intensive interventions for Mental Health and substance use issues as well as housing assistance
- Attend and participate in assigned staff meetings and with other personnel involved in the program to fully disseminate and receive information on individual clients that will benefit the overall support services being provided
- Write reports, properly document, and compile statistical data, including monthly, quarterly, and annually
- Shop for food and other supplies for residents, community dinners/lunches/breakfasts, Morning Market, etc.
- Other related duties as required
EDUCATION AND EXPERIENCE:
- 21 years of age (required)
- CA Driver’s license (required)
- Master’s degree (preferred)
- Experience in counseling, case management, and assessments
- Bachelor’s degree in Social Work, Psychology, Counseling; or related social sciences field and 5+ years of experience in Mental Health and substance use disorders, counseling and assessments, case management
SKILLS AND ABILITIES:
- Knowledge of alcohol and other related drug, the 12-Step program, and other supportive treatment programs and groups
- Knowledge of community resources and linkages to substance abuse recovery and Mental Health communities, including homeless population and veteran- and Youth-based service providers
- Knowledge of Microsoft Word and Excel computer programs
- Ability to maintain the highest level of integrity and confidentiality
