Description

Larkin Street Youth Services is a nonprofit empowering young people to move beyond homelessness.  Founded in 1984, Larkin Street has helped over 75,000 young adults in San Francisco by providing a continuum of healthcare, housing, employment, and education services.  Larkin Street is also part of the movement to end youth homelessness on a national scale through policy partnerships with national organizations and fellow nonprofits.  Our services are tailored to meet young people where they’re at, making our programs easy to get into and hard to fall out of, with the ultimate goal of helping young people develop the knowledge and skills needed for independent adulthood.

In addition to two short-term shelters, Larkin Street offers more than 400 housing beds for up to two years or more. More than just safe places to sleep, our housing programs offer comprehensive supports to young people, including individual/group counseling, life skills trainings, and more. Some provide 24-hour onsite staffing, while other programs give young people the chance to live more independently in their communities.

 

PRIMARY RESPONSIBILITY:

The primary role of the Case Manager II is to provide comprehensive strength-based case management services to homeless and/or marginally housed transitional aged youth. The Case Manager II is responsible for the comprehensive evaluation of bio-psychosocial, emotional, educational, vocational and functional needs of young people; develops an individualized service plan in coordination with Larkin Street Youth Services continuum of care as well as leverages relevant community resources as needed.  The Case Manager II leads collaborative service planning and must have a thorough understanding of the stages of change and development. The Case Manager II must be comfortable providing services to clients outside of normal office settings, including but not limited to emergency shelter environments, congregate living housing placements, scattered site housing, and single room occupancy (SRO) placements throughout the city of San Francisco.

 

CORE JOB FUNCTIONS:

  • Is responsible for intake and assessment; direct service linkage and referral; case planning and contracting; case coordination and advocacy; and follow up and after care
  • Provides crisis prevention, intervention, and safety planning services, along with stress management and life skills development
  • Assists young people in applying for and obtaining benefits and housing
  • Assists in providing client resources of outside agencies and supportive services that promote the health and wellness of youth
  • Works closely with community behavioral health providers and internal service teams to ensure coordinated and effective services
  • Maintains accurate detailed observation and progress records and completes necessary electronic data entry into agency and city databases.  Identifies trends in data to assist in collaborative service planning
  • Adheres to laws regarding confidentiality (HIPAA) and reporting requirements and maintains necessary compliance documentation
  • Develops and maintains good relationships with clients
  • Collaborates with fellow staff members and established community partners, including property managers and landlords
  • Facilitates transportation and provides accompaniment to client appointments in the community
  • Completes safety and wellness checks regularly in residential units
  • Teaches life skills (e.g., cooking, cleaning, budgeting)
  • Participates in organizational meetings (e.g., all-staff, CQI, Client Practice, etc.)
  • Provides services when and where clients need them; evenings and weekends will be required when program needs dictate
  • Responsible for supporting program services within the milieu.  For example, crisis intervention or helping to maintain the safety of the program
  • Responsible for contributing towards the care and welfare of the clients we serve
  • Other duties as assigned

Qualifications

EDUCATION & QUALIFICATIONS:

  • Bachelor’s degree in related field.  Years of experience can be considered in lieu of degree.
  • 3 to 5 years’ Experience working with youth. Experience working with high-risk youth in a shelter and/or transitional/supportive housing program is highly desirable
  • Working knowledge of issues facing homeless and at-risk youth
  • Ability to work with diverse staff, clients, and volunteers
  • Cultural humility and familiarity with the diversity of transgender, lesbian, gay, and bisexual identities, and experiences
  • Ability to identify, assess and intervene effectively with mental health symptoms and behaviors
  • Experience working with strength-based, restorative justice, trauma informed care practices, and client centered models
  • Valid California driver’s license with clear DMV record
