SAFY is seeking a full time Case Managerfor our Huntsville division in Alabama.
As a Case Manager, you will be responsible for the case management functions of families and youth in our division.

What You’ll Do
- Provide Therapeutic Foster Care case management to families and children on your caseload.
- Complete referral reviews and intake assignments. Communicate service availability to all assigned clients promptly.
- Provide instruction, intervention and assistance as deemed necessary through the exercise of independent judgment and knowledge or as part of the treatment team to preserve the family unit.
- Coordinate with county agency as appropriate and attend family sessions to promote successful clinical interventions with foster youth.
- Collaborate service provisions with school officials, courts and other appropriate parties involved with the client.
- Provide Medicaid billable services to the youth with mental health diagnosis; promote daily living skills that impact delivery of mental health services which focus on self-sufficiency through specific outcomes. This may include the following: obtaining housing, job education/vocational training, or counseling and community networking.
- Complete assessments as required by state rules, agency policies, and/or contract requirements.
- Prepare Individualized Treatment Plans, DAF's, and Mental Health Services Plans according to established time frames involving all appropriate parties as required by policy and regulations of the state.
- Timely document outcome of home visits, support sessions, interaction with parties involved in the youth's care and incidents as required by policy, regulations and contract requirements.

Qualifications
- Bachelor's or master’s degree in the field of social work, counseling, psychology, sociology, child & family studies is required.
- Licensure in the State of Alabama in highly preferred
- Three years of relevant experience is required with above listed degrees and experience in child welfare preferred.
What We Offer
- Targeted hiring range: $36,000 - $45,000
- Compensation is commensurate with transferrable experience, education and licensure of candidate, along with internal equity and budgeted amount for the role.
