Salary range:
Case Manager I: $40,809.60 Annually
Case Manager II: $43,305.60 Annually
Case Manager III: $46,009.60 Annually
Shift available: Full-time
Essential duties and responsibilities:
- Staff will maintain ethical and professional communications at all times using respectful Trauma-Informed Care language
- Serves as a Care Coordinator for adults in the Hereford service area experiencing behavioral health issues
- Will be responsible for organizing consumer activities and sharing information among all participants concerned with the client's care to achieve safer and more effective recovery
- Will conduct state-approved assessment and determine treatment recovery plans with individuals' involvement
- Will connect client to identified community resources, provide follow-up with referrals and document activities according to agency policy
- Position will organize and provide culturally competent care activities and collaborating among all participants involved with the individual's care and treatment
- Additional duties can include: assessment & treatment/recovery planning, working within the team to ensure appropriate services are delivered, engaging individual by taking a person-centered approach to help identify strategies and objectives
- This position will spend at least half of his/her working time directly working with the individuals assigned to caseload
- Position will assist & collaborate with the interdisciplinary Behavioral Health Team in promoting recovery, skill development and community stability
- Adheres to TPC Business Code of Conduct and reports concerns if ethical and/or business standards defined in the code are compromised
Required qualifications to be successful in the job:
- Bachelor's degree from an accredited university with a major in psychology, sociology, social work, counseling, or other related human services field for QMHP credentialing
- For Case Manager I, no experience required
- For Case Manager II, one (1) year experience of behavioral health case management
- For Case Manager III, two (2) years experience of behavioral health case management
- Graduate work in a related field may be substituted for up to one (1) year of experience
- Must have a valid Texas driver's license and be insurable under agency vehicle policy
- Must be 18 years of age or older
Preferred qualifications to be successful in the job:
- Bilingual
- Degree in social work or psychology
