We currently seek a full-time Facility Linkage Specialist to join our Criminal Health Initiative team! The Facility Linkage Specialist is responsible for working in New York State prisons to deliver linkage navigation & health education services to individuals who are HIV/HCV Positive. The Facility Linkage Specialist will also provide HIV counseling sessions to incarcerated participants at contracted facilities. 


Salary Range:   $30.41 - $31.77 hourly 


Essential Duties:

  • Responsible for meeting with participants and offering Linkage Navigation Services (reentry case management) to HIV/HCV positive people who are due to be released from prison
  • Conducting a comprehensive Intake/Linkage Action Plan (reentry case management plan) that will assist the participant with their Reentry Process
  • Responsible for delivery of program services including recruitment of participants into Osborne Programs (IDI, IDG, LNS, etc.)
  • Promote, conduct, and coordinate health education groups, Interventions delivered To Individuals (IDIs), Interventions Delivered to Groups (IDGs), and Criminal Justice Initiative (CJI) Peer training classes
  • Providing individual support treatment counseling (ARTAS) with participants who are not adhering to their HIV and/or HCV treatment regimens
  • Coordinate with DOCCS's Offender Rehabilitation Coordinators (ORCs), Nurse Administrators (NAs), Regional Infectious Control Nurses (RICNs) and Senior Utilization Review Nurses (SURNs) regularly to obtain medical documentation
  • Coordinate transportation home from New York State prisons for all participants who are HIV and HCV positive
  • Appropriately record information in participant charts related to all health education, peer, ARTAS, and Linkage and Navigation Services offered
  • Responsible for keeping abreast of the latest information about HIV/AIDS, Hepatitis and other co-infections that may occur with HIV disease and is familiar with various services available in the community and can speak knowledgeably about such issues when they are discussed in counseling sessions as well as in group presentations
  • Responsible for meeting all AIDS Institute contract training requirements
  • Connect with community partners (CJI community providers, clinics, substance treatment programs, etc.) to assure a strong and continued successful working relationship
  • May have to facilitate the various aspects of their responsibility through video calls or phone calls with DOCCS Prison Staff and incarcerated individuals
  • Scheduling doctors appointments, HRA meetings, and providing escorting HCV+ participants to and from various Social Services Agencies and providers as needed
  • Responsible for recording accurate information in participant charts related to intake and assessment, individual level interventions, referrals and other services
  • Enter data into the AIRS System
  • Adhere to guidelines set forth in the policies and procedures manual and adhere to prison policies and procedures
  • Responsible for coordinating services and must maintain constant communication with the Program Manager to assure that the day to day operations are being conducted accordingly
  • Attend assigned training 
  • In-person attendance is required to collaborate with co-workers
  • Perform other duties as assigned


Requirements

 Minimum Qualifications:

  • Bachelor's Degree in Human Services, Sociology, Social Work or related field; in lieu of a degree, Highschool Diploma or equivalent with an additional two years of related experience in a social service organization
  • Two years of experience in a social service organization, supporting participants and providing case management

Key Competencies:

  • Ability to travel locally  and to assigned New York State prison facilities 
  • Proficient in Google Suites and utilizing a database management system
  • Ability to work effectively under tight deadlines, strong attention to detail, and time management skills
  • Knowledge of the criminal justice system
  • Excellent assessment, treatment planning and conflict resolution skills 
  • Ability to work in teams and collaborate effectively with people in different functions
  • Ability to take the initiative and drive for results
  • Ability to communicate with all levels of the organization and build strong relationships
