Responsibilities:
- Provide case management services to individuals and families in need
- Conduct assessments to determine client needs and develop appropriate care plans
- Coordinate and monitor services provided by various agencies and organizations
- Advocate for clients' rights and access to resources
- Collaborate with other professionals, such as social workers and healthcare providers, to ensure comprehensive care
- Maintain accurate and up-to-date case records
Experience:
- Minimum of 1 years of experience in case management or related field
- Familiarity with workforce development programs and resources
- Understanding of behavioral health issues and crisis management techniques
- Proficiency in crisis intervention strategies
Skills:
- Strong communication and interpersonal skills
- Ability to build rapport with diverse populations
- Excellent organizational and time management skills
- Critical thinking and problem-solving abilities
- Ability to work independently as well as part of a team
- Proficiency in computer applications, including Microsoft Office suite
