Responsibilities:
- Provide case management services to individuals and families in need
- Conduct comprehensive assessments to determine client needs and develop appropriate service plans
- Coordinate and monitor the delivery of services to ensure client progress and goal attainment
- Collaborate with community resources and agencies to provide necessary support and referrals
- Advocate for clients' rights and access to services
- Maintain accurate and up-to-date case records and documentation
- Provide crisis intervention as needed
- Utilize motivational interviewing techniques to engage clients in the change process
- Supervise and mentor other case management staff
Qualifications:
- Bachelor's degree in social work or a related field (Master's degree preferred)
- Experience in case management, social work, or a related field
- Knowledge of Medicare, Section 8, child welfare, addiction counseling, and crisis management
- Strong communication and interpersonal skills
- Ability to work effectively with diverse populations
- Excellent organizational and time management skills
- Ability to prioritize tasks and meet deadlines
- Proficiency in computer applications and case management software
