DUTIES AND RESPONSIBLITIES
Carries out case management activities as per the Part 900 Regulations and the Department of
Homeless Services (DHS) Case Management Guidelines for family shelters
Completes an assessment and fathers required documents (i.e.: birth certificates, social security cards,
picture IDs, etc.)
Shows the Safe Sleep Video and carry out the appropriate documentation process
Makes sure that clients sign the Code of Conduct and the facilities rules and regulations
Obtains any possible proof of employment and assists with obtaining entitlements
Maintains clients' records by reviewing case notes; logging events and progress
Communicates clients' progress by conducting weekly interdisciplinary meetings and evaluations
Handles intakes of adults and ensures that all documentation is accurate and properly filed
Recommends action plan and develops an Independent Living Programs (ILP) for clients that is
reviewed and updated on a weekly basis
Collaborates with the Housing Specialist to create an exit strategy and target exit date
Tasks family with seeking and accepting housing
Responsible for meeting target goals set for each client by the Housing Specialist
Conducts weekly unit inspections
Maintains detailed records of each client in the form of progress notes through CARES
Provides Director of Social Services (DSS)/ Case Manager Supervisor with reports and summaries of
caseload for management review
Liaison for management, communicating clients' progress and any case concerns
In the event of an emergency, Essential Employees are required to report to work as scheduled
Duties and responsibilities are subject to change based upon assessment of company needs
SKILLS AND QUALIFICATIONS
Bachelor's Degree in Psychology, Social Work, Sociology, Mental Health, Nursing, Human Services, or
Education Services required.
Minimum of 2 years' experience working within the Social Services Field
Must have strong interpersonal skills and be able to communicate effectively (verbally and in writing)
Must be organized
Experience with DHS Cares / Homeless population is a plus
Bilingual English/Spanish is a plus
