ESSENTIAL JOB RESPONSIBILITIES
1. Meet with residents and families regularly
2. Complete a social assessment and develop and coordinate an Individualized Service Plan (ISP) on all residents.
3. Help residents make the adjustment to their new home.
4. Develop a formal and informal flow of communication and cooperation with all other staff members who serve the resident.
5. Be involved with and provide support to residents and families when transfer to an alternative care setting is necessary.
6. Provide input and assistance to the activity program.
7. Participate in community activities involving the mentally ill.
8. Provide counseling services to residents in the event of crises in their lives.
9. Work with residents to identify care needs and ensure they are met.
10. Work in conjunction with the Administration to arrange for an aftercare program which is sufficient to the residents’ needs.
11. Work with other resource people to coordinate resident services, located inside or outside the Facility. These may be provided by formal or informal helpers (i.e. staff, professionals or family members and volunteers).
12. Note and notify the appropriate authority if there is a change in the resident’s mental or physical status.
13. Develop brief case records on each resident upon admission (initial), and then when incidences warrant, also annually, to provide continuity of care.
14. Arrange for service in areas of income maintenance, health, mental health, and social service. Assist residents as they apply for income and public benefit entitlements.
15. Develop a resource file on services, benefits and entitlements for residents.
16. Visit residents when they are in the hospital
17. Maintain updated records on residents’ benefits (i.e. Social Security, SSI, HR, Pensions, Medicaid, Medicare etc.)
18. Ensure that Medical benefits are current and in effect.
19. Facilitate the remembering of birthdays, anniversaries, and illnesses and deaths with cards.
20. Assist with the resident council.
21. Assist the Administrator as a representative of the Facility in the community.
22. Review residents service needs periodically and acts as their spokesperson when necessary. Note complains and assist in their resolution.
23 Ensure that all residents’ have an updated Medical and Mental Health Form filled out on an annual basis and when there is a change in the medical condition.
24. Carry out other responsibilities as assigned by supervisor
QUALIFICATIONS
1. B.A degree from an accredited college or university with major work in human resource or service delivery.
2. Minimium of one year experience, with the provision of services, to a dependant adult population.
3. Flexibility and creativity in dealing with the issues being faced by esidents and families
4. Ability to exercise independent judgment within the rganization's mission and philosophy
5. Experience and enjoyment in working with older adults
