Position Summary:
The Case Manager is responsible for providing case management services to students and their families; initial and on-going collateral work with referral sources, guardians, courts, school systems, coordination of internal services that includes facilitating and the development of each students Individual Education Plan and Individual Service Plan. The Case Manager will work as part of the educational and treatment team. They are the primary advocate for service excellence as it relates to the students on their caseload. They are also responsible for coordinating all processes related to academic, residential, programming, medical and transition services.

Education and/or Experience
- Associate’s degree in related field of study
- Minimum of two years human service experience preferred
- Prior experience working with students with disabilities
Essential Functions:
- Work with funding agencies and families in the best interest of the student and community and alerts appropriate administrator(s) to identify potential conflicts
- Ensure telephone and mail contact with families or other identified supportive adults in accordance with the individual service plan and program rules and standards
- Maintains professional boundaries and confidentiality with families, guardians, and other external stakeholders
- Communicate education and treatment progress, issues, concerns, and general status to families, and referral source and other relevant external case workers as needed, and minimally once every two weeks
- Ensure adequate and on-going communication of education and treatment progress, issues, concerns, and general status to all members of the Crotched Mountain Team
- Provide initial response to most concerns and complaints from parents, guardians, workers, and other external stakeholders, working closely with the residential and education administrative team
- Attend relevant off-campus case and administrative reviews, court reviews and hearings, foster care reviews, hospital discharge planning meetings, and other off-campus meetings designed to ensure clear communication and cohesive service delivery
- Attend all required treatment team, residential, educational, case management, and campus-wide meetings
- Promote wraparound care and coordinate parent/guardian training to support student growth
- Other duties as assigned
Job Type: Full-time
Pay: $52,000.00 - $56,000.00 per year
