PURPOSE OF POSITION
RESPONSIBILITIES
The major responsibility of this position includes, but are not limited to:
- Completes initial assessment as needed and reassessments of all assigned residents in a timely manner in-order to develop individualized treatment goals for each person
- Establishes treatment goals and objectives with residents as part of the interdisciplinary team.
- Provides case management services including but not limited to weekly and monthly progress summaries, quarterly assessments, quarterly care planning, counseling, outreach, case conferences, advocacy, and referrals.
- Ensure timely submission of clinical documentations to care organizations for uninterrupted services.
- Maintain documentation and update care plans at specific time intervals or as needed.
- Accompanies residents to HIV primary care and supportive service appointments as needed.
- Create progress notes documentation and service coding according to program Scope of Services with established deadlines.
- Understands and educates clients on chronic medical conditions, including HIV, mental health, substance use, diabetes, asthma, hypertension, heart disease and hepatitis diseases.
- Collect biomedical, psychosocial, and award letters on a quarterly basis.
- Conduct monthly in-person wellness checks.
- Facilitate monthly support groups.
- Participate in performance improvement activities utilizing principles to support and improve resident care.
- Keep current with literature regarding changing practices, interventions, and best practice.
- Communicate accurately and appropriately (responding to emails in a timely manner).
- Collect and submit all rental and electric bills monthly.
- Performs other duties, consistent with the goals and objectives of the program, as directed by the Program Manager.
- High School Diploma/GED
- Associates Degree preferred.
- Minimum one (1) year of similar work experience, facilitating groups case management or housing experience.
- Previous work experience providing case management to PLWHA.
- Outstanding writing and communication skills.
- Knowledge of citywide resources and referral processes for PLWHA
- Proficient with Microsoft Office Suite and related program software.
- Ability to effectively communicate with individuals served, management & other stakeholders.
- The base pay range for this position is $40,000 - $45,000 per year. *
