Case Manager Position Description

Schedule: Friday, Saturday, Sunday, Monday 8:30-5pm

You can choose which two days you want off between Tuesday, Wednesday, and Thursday.

This position will be in Placentia, California.

Essential Functions

Maintaining Units:

• Distributing supplies to clients
• Ensuring units are supplied with all necessary items
• Cleaning units after move out in preparation for next move in
• Preparing units for move in

Provide direct case management services to client’s that includes:

• Guidance in planning for future goals and setting attainable objectives.
• Providing support, encouragement, and accountability to clients in their daily activities toward reaching the goal of self-sufficiency.
• Conducting random client drug tests.
• Providing substantial client direction regarding essential life skills such as budgeting and the monitoring of client’s savings accounts.
• Client referrals to community resources.
• Functioning as a liaison between client and external agencies.
• Providing conflict management, crisis intervention, and enforcement of program rules and regulations with each client as needed or required.
• Supervising House Manager regarding enforcement of house rules; assignment of chore duties, laundry schedule, visitors policy, zero tolerance drug/alcohol policy, general cleanliness of units and exterior premises.
• Conduct client exits of guests according to established policies of Colette's Children's Home.

Additional Responsibilities

• Produce reports as specified by governmental funding requirements or requested by Colette's Children's Home management.
• Attend training classes or meetings as required or as needed to maintain overall program integrity, individual client success, and fulfill agency requirements.
• Maintain communication with CCH Program Supervisor and other staff members by attending weekly staff meetings and completing written reports when necessary.
• Data Entry
• Organization and cleaning of property

Job Type: Full-time

Pay: $21.00 - $23.00 per hour

Expected hours: 40 per week

Education:

• Bachelor's (Preferred)

Experience:

• Social Work: 1 year (Required)
• working with the homeless population: 1 year (Preferred)

License/Certification:

• Driver's License (Required)

• Suite 106Huntington Beach, CA 92647: Relocate before starting work (Required)
