The Case Manager functions as the primary liaison between the treatment team and parent regarding coordination of services. The Case Manager will work closely with the childrens services program team to optimize the Quality of Life (QofL) of the students served by Anderson. The Case Manager promotes and stewards the sense of family in all connections related to Anderson. The Case Manager is responsible for the coordination and documentation of all student specific services.
Pay Range: $22.12 - 24.04 houlry (based on years of related experience)
RESPONSIBILTIES:
- Under the direction of the Family Liaison, the Case Manager coordinates the provision of all activities, services, and supports identified by the IEP and Individualized Plan of Protective Oversight (IPOP).
- The Case Manager serves as principal liaison between the treatment team and parents/guardians.
- The Case Manager works closely with the Admissions Department and program teams to facilitate successful student transition upon admission and discharge.
- Under the direction of the Family Liaison, the Case Manager ensures student records are maintained in accordance with all regulatory guidelines and appropriate documentation is maintained in Therap.
- The Case Manager participates as an active member of the Interdisciplinary Treatment Team.
- The Case Manager is an active, engaged, and collaborative member of the Development Department.
- The Case Manager continues professional development through information sharing with colleagues, attendance at in-service programs and conference/workshop attendance.
REQUIREMENTS:
- BA in Human Services with sufficient experience to perform the responsibilities of the position preferred.
- Ability to use Microsoft 365 platform preferred.
- Flexible schedule, nights and weekends may be required.
- Must maintain qualifications in the crisis intervention procedure of the agency.
- Must be capable at all times of initiating or assisting in all components of the crisis intervention of the agency.
- Must be capable at all times of assisting occupants in the orderly evacuation of the facility, by both primary and secondary paths in the event of fire or other emergency.
