Position Type:    Full Time

Pay:                  $23.78/hr.

About this opportunity:

Interviews, accepts and provides comprehensive, long-term, structured, complex Case Management services for an assigned caseload of clients participating in an established life management program; understands the uniqueness of the clients history in order to determine the most effective program plans, develops comprehensive program plan/goals and evaluates client's progress by conducting mentoring and counseling sessions with client and/or staff; serves as advocate for client in order to acquire services that will enable them to functionally cope with their environment, ensures constant compliance with funding requirements.

Key responsibilities:

  • Provide Case Management services to assigned caseload at the Center of Hope after clients complete the Coordinated Intake process.
  • Assess each client for supportive service needs focusing on the following nine areas of concern: Health, Housing, Substance Abuse, Mental Health, Employment, Legal, Benefits, Financial Literacy and Domestic Violence.
  • Evaluate/measure client progress toward their successful completion of the goals and objectives identified in the service plan. Evaluation should also include other service providers and significant others.
  • Connect clients to appropriate income assistance resources and/or assist clients with the completion of employment and disability applications when needed.

 

What we are looking for in you:

  • Bachelors degree from an accredited college or university in Social Work; Masters preferred AND
  • Three years of experience in Case Management.  Background working with the homeless population a plus OR
  • Any equivalent combination of training and experience which provides the required knowledge, skills and abilities.

  

Physical Requirements and Working Conditions:

Work in a normal office environment where there are little or no physical discomforts associated with changes in weather such as rain, cold, snow, heat or discomforts associated with noise, dust, dirt, and the like.  Duties are usually performed seated.
