Job Description
The Case Management Coordinator is a member of the Case Management team and works to improve the health outcomes of our patients. This position assists the case managers and social workers to ensure patients receive excellent transitions of care and appropriate continuum of health care. Using reports, both internally and externally, this role conducts patient outreach to assist patients with scheduling, ensures patients have appropriate follow up instructions, and connects patients with support services agencies as needed. The position is involved in activities to engage the patients in ongoing efforts of continuous transitional care and coordination, including insurance authorizations follow up.
Responsibilities
- Uses appropriate internal and external reports to conduct outreach to post-discharge patients for the purpose of care coordination.
- Communicates with Case Management, physicians and APPs regarding care coordination activities or follow ups.
- In collaboration with Case Management, prioritizes clinically important conditions for care coordination focus.
- Manages the care coordination effort to ensure providers are aware of key patient events: readmissions, ED visits, and adverse events.
- Facilitates patient access to key clinical and non-clinical post-discharge to enhance patient experience and reduce overall health system and payer resource consumption.
- Develops and sustains excellent relationships and communications with our primary care physicians, APPs, practice managers and staff.
- Works with appropriate in-patient and long-term care departments and staff to develop and maintain excellent transitions of care processes.
- Assists in departmental expense validation and reconciliation.
- Follows up on patient insurance authorization status to facilitate patient discharges.
Qualifications
- High School Diploma or Equivalent Required.
- 2 years customer service experience Required.
- Excellent problem-solving skills.
- Excellent communication skills.
- Computer skills working with Microsoft Word, Excel, and PowerPoint.
- Basic knowledge of medical terminology, anatomy and physiology.
