Giving Home Health Care is eager to find motivated and caring individuals to be advocates for our clients. In this role you will be a valued member of a Care Team, work with our clients and contractors to provide information about the program that we work with, and provide a personalized experience for each client.
We are looking for someone with great communication and time management skills to review patient charting submitted by contractors to ensure our patients are receiving the best quality of care.

Responsibilities- Working on new patient admission packages and recertification of current patients
- Helping and preparing plans of care according to Dr.'s orders
- Daily Patient Case Management documentation and record-keeping
- Periodic home visits to better serve our patients
- Accompanying patients to doctor appointments to advocate and explain program requirements
- Weekly conversations with patients and caregivers
- Requesting and researching medical records
- Are organized, well-disciplined, and able to manage multiple projects
- Have strong communication skills
- Display a warm and friendly attitude to everyone
- Have excellent reading, writing, and computer skills
- Have and follow a well-developed value system, are loyal, and interested in serving humanity
- Can quickly learn and use various computer programs and applications
- Are familiar with medical terminology and/or have recent experience in the medical field
- Understand and have been involved with maintaining confidentiality and compliance enforcement
