Alternatives' professionals are connecting with ONE IN FIVE individuals over the age of 60 in a sixteen-county region.
BASIC PURPOSE/SCOPE:
Assists to assure for the health, safety and well-being of older adults in sixteen counties on behalf of Alternatives' mission. Responsible to perform intake, data entry, maintain records and billing for individual data. Provides administrative support to agency programs and services such as: Care Coordination, Adult Protective Services, Ombudsman, Money Management, Caregiver Support and Bridges Community Center.
BASIC RESPONSIBILITIES:
1. Telephone Reception and Response to Visitors.
Response to callers and visitors is courteous, helpful, and professional. Answers inquiries, knows resources, and gives accurate information.
2. Intake.
Gathers intake information on client referrals for Care Coordination and Adult
Protective Services (APS). Referral is complete and information correct. Assigns and distributes referrals to the proper Care Coordinator, Caregiver Counselor, or APS Caseworker and enters information into the proper database.
3. Process Paperwork.
Processes paperwork for new applications, annual paperwork, Redeterminations, Temporary Service Increases, Follow-ups to Interim Services, and Nursing Home Prescreens. Completes data entry and mails appropriate paperwork to vendor and client. Processes paperwork for Respite, Caregiver and Supplemental cases. Processes APS paperwork for billing.
4. Care Coordinator Support
Prepare paperwork for submission with participant assessment information. Assist to track outstanding paperwork or needed information in order to process. Share phone call information with relevant care coordinator.
5. Management of individual Services.
Performs duties related to management of individual services, includes notifying vendors with individual service information, obtaining effective dates, and documenting files appropriately.
6. Record Maintenance.
Initiates and terminates files. Maintains files in an orderly manner and in appropriate place. Files individual information and files in the central filing system.
7. Quality Assurance.
Participates in Peer Review and Continuous Quality Improvement file review.
8. Other Duties as Assigned.
Distributes faxes, makes copies, completes end of day tasks and assists to assure smooth operation of services and completes other tasks as assigned in support of Alternatives' mission.
JOB REQUIREMENTS:
Minimum of a high school diploma or equivalent. Two years education and/or experience in the clerical field preferred. Computer skills. Knowledge of general office procedures and equipment. Excellent communications, problems solving and organizational skills. The ability to maintain a pleasant manner with the public and vendors at all times.
SUPERVISION: The person will be responsible to the Data and Support Supervisor
