Qualifications for CareManager:
Assistsall patients through the healthcare system by acting as a patient advocate andnavigator, able to guide the client and family with a road map to the qualityof life.
Coordinatescontinuity of patient care with external healthcare organizations andfacilities, including the process hospital admission and discharge andreferrals from the primary care provider to a specialty care provider.
Evaluatesclinical care, utilization of resources, and development of new clinical tools,forms, and procedures.
Supportand closely coordinate health care with the patient's primary care providerand, when appropriate, other treating providers.
Screenand assess patients by completing a psychosocial assessment and develop a careplan.
Providepatient education about common disorders and available treatment options.
Supportpsychotropic or general medication management prescribed by PCPs, focusing ontreatment adherence, side effects and other complications, and effectiveness oftreatment.
Documentpatient progress and treatment recommendations in the system so they can beeasily shared with PCPs, the psychiatric consultant, other treating providers,and family members.
Facilitatetreatment plan changes for patients who are not improving as expected inconsultation with the PCP and the psychiatric consultant. These may includechanges in medications or psychosocial treatments or appropriate referrals foradditional services.
Facilitatereferrals for clinically indicated services outside the primary care clinic(e.g., social services such as housing assistance, vocational rehabilitation,mental health specialty care, substance abuse treatment).
Requirements for CareManager:
Currentunrestricted RN License or LCSW/MSW or Advance degree in Psychology, SocialWork or Nursing
Abilityto work with patients by telephone or in person.
Experiencewith assessment and treatment planning for common health disorders, whenappropriate.
Workingknowledge of differential diagnosis of common mental health and/or substanceuse disorders, when appropriate.
Familiaritywith brief, structured intervention techniques (e.g., MotivationalInterviewing, Behavioral Activation), when appropriate.
Demonstratedability to collaborate effectively in a team setting
Reliabletransportation and insurance
Ableto receive direction and independently follow tasks through with minimumsupervision.
Detailedorientated, able to multi-task and remain flexible with assignments.
Abilityto maintain effective and professional relationships with patient and othermembers of the care team.
