Responsibilities:
- Provide comprehensive care management services to individuals and families in need
- Conduct assessments to identify client needs and develop individualized care plans
- Coordinate and monitor services provided by a multidisciplinary team of professionals
- Collaborate with community resources to ensure clients have access to necessary support services
- Advocate for clients' rights and ensure they receive appropriate care and services
- Provide crisis intervention and support during emergency situations
- Utilize motivational interviewing techniques to engage clients in their care
- Supervise and mentor junior Care Managers
Requirements:
- Bachelor's degree in social work or a related field (Master's degree preferred)
- Experience in behavioral health, child welfare, or a related field
- Strong knowledge of Medicare, Medicaid, and other public assistance programs
- Familiarity with Section 8 housing assistance program
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
