The Care Coordinator is responsible collecting and documenting all required data to manage a patient’s episode of care from end to end. This requires identifying the need(s) of the referring source and/or patient, collecting all necessary data relevant to their care and entering data to the appropriate clinical databases. The Care Coordinator must also work to meet contractual and regulatory turnaround times for their assigned patient cases. The Care Coordinator actively engages and coordinators with other team members and clinical staff to maintain a positive, collaborative, patient-centered relationship.

Training & Nesting Hours – Monday – Friday 8:00am – 5:00pm EST
- Monday-Friday 11:00a-8:00p EST
- Tuesday-Saturday 9:30am-6:00pm EST
- Sunday-Thursday 9:00am-6:00pm EST
- Collect, verify, and confirm non-clinical information, including the management of outbound phone calls necessary to support the clinical program.
- Learn, understand and maintain working knowledge of products and services offered by the company
- Maintain adherence to operational processes to ensure compliance to company policies, legal requirements, and regulatory mandates
- Meet timely and accurate documentation needs to ensure the success of CareCentrix and the needs of the clinical program
- Work with the clinical team to prepare forms and other necessary documentation for delivery to Medical Directors and/or external providers to secure clinical information needed to plan and implement clinical program obligations.
- Provides appropriate issue resolution and/or escalation when needed. Works under close supervision.
- High School Diploma or the equivalent plus a minimum of 1 year medical terminology or medical services experience required.
- Minimum of 2 years insurance, quality improvement or healthcare operations required.
- Must have knowledge of basic spreadsheet, word processing, and data entry skills.
- Experience in a call center environment preferred.
- Excellent communication and problem solving skills, as well as the ability to interact with all levels of management and a highly diverse clientele.
- Starting Pay for external hires is $18.00/hr + Incentive Bonus Opportunity. The pay range included in this posting reflects future growth / earning potential.
