Care Coordinator, Registered Nurse
Purpose
This position coordinates care for Medicare patients in our community, supporting them with Transitional Care Management, Chronic Care Management, Annual Wellness Visits and health education. A nurse in this position will work very closely with our nursing, healthcare providers and social work to support our patients with a well-rounded care team.
Duties and Responsibilities
- Coordinates continuity of patient care with both internal and external healthcare organizations and facilities.
- Conducts comprehensive/preventative screenings with patients at their visit
- Supports patient self-management of disease and behavior modification interventions
- Manage high risk patient care, including management of patients with multiple comorbidities or high risk for readmission to a hospital setting
- Support nursing staff when needed for triage call coverage and prescription refills during clinic hours
- Participate in community engagement events
Qualifications
- Registered Nurse, Bachelor’s Degree preferred however will accept an Associates Degree for the right candidate
- BLS required, ACLS/PALS is a plus
- Previous experience in Medicare Care Coordination, or work in a Rural Health Clinic encouraged, but not required

Being a part of the Rural Health Clinic program, we deliver high quality and needed care for our rural area. Along with our Internal Medicine team we have integrated a Behavioral Health practitioner to support the growing need in our area. Additional specialties include: General Surgery, Podiatry, Endocrinology, Cardiology, and Gastroenterology.
