Form of workCare Coordinator, Registered Nurse
Purpose
This position coordinates care for Medicare patients in our community, supporting them with Transitional Care Management, Chronic Care Management, Annual Wellness Visits and health education. A nurse in this position will work very closely with our nursing, healthcare providers and social work to support our patients with a well-rounded care team.
Duties and Responsibilities
- Coordinates continuity of patient care with both internal and external healthcare organizations and facilities.
- Conducts comprehensive/preventative screenings with patients at their visit
- Supports patient self-management of disease and behavior modification interventions
- Manage high risk patient care, including management of patients with multiple comorbidities or high risk for readmission to a hospital setting
- Support nursing staff when needed for triage call coverage and prescription refills during clinic hours
- Participate in community engagement events
Qualifications
- Registered Nurse, Bachelor’s Degree preferred however will accept an Associates Degree for the right candidate
- BLS required, ACLS/PALS is a plus
- Previous experience in Medicare Care Coordination, or work in a Rural Health Clinic encouraged, but not required

We serve 28 towns in the Woodsville/North Haverhill area both in New Hampshire and Vermont. Being a part of the Rural Health Clinic program, we deliver high quality and needed care for our rural area. Along with our Internal Medicine team we have integrated a Behavioral Health practitioner to support the growing need in our area. Additional specialties include: General Surgery, Podiatry, Endocrinology, Cardiology, and Gastroenterology.
