PORTIA BELL HUME BEHAVIORAL HEALTH AND TRAINING CENTER (aka The Hume Center) has a rewarding opportunity for a qualifiedCare Coordinator: Substance Use Specialistunder the leadership of The Hume Center’s Clinical Program Manager This is an exciting opportunity to help our organization to grow by continuing to develop this department.

 

MISSION: The Hume Center strives to create an environment of psychological well-being both internally for our work community and externally for the community we serve, with special consideration for the under- and un-served, through practicing an enabling consultation process. We believe in serving our community through strengthening existing and developing new resources; providing the highest caliber of culturally sensitive behavioral health services, professional training, and clinical research; and focusing on consistent improvement through outcomes, data-based quality assurance, and program evaluation.


MINIMUM REQUIREMENTS
A. CADC Registration or Certification OR Licensed Mental Health Clinician with Substance Abuse Training
B. Experience with Integrated Dual Disorder Treatment (IDDT)
C. Two years’ relevant behavioral health and Substance Use disorder clinical experience.
D. Knowledge of psychiatric disability and the special treatment needs of consumers with mentalillness and co-occurring Substance Use.
E. Speaking, writing, and reading fluency in Spanish, Vietnamese, or Laotian is preferred but not required.
F. Experience at The Hume Center is a plus but not required
G. Demonstrate flexibility to handle diverse and changing organizational needs
H. Must be willing to work full-time, minimum 40 hours/week, five days a week, and be present at The Hume Center.
I. Must be willing to work some evenings and weekends when needed.
J. Demonstrate primary professional commitment and knowledge for representation of The Hume Center.
K. Experience working effectively with culturally, socio economically, and ethnically diverse populations.
L. Strong commitment to respecting consumers and their family members and having concrete ideas about how to improve the meaningful involvement and participation of consumers in the program’s services.
M. Strong commitment to community-based services for persons with high needs and low resources.
N. Some knowledge of psychiatric disability and the navigation of the system of care for those with mental illness in the local area
O. Familiar with and/or able to work within the Family Partner and Consumer Empowerment shift in Psychology.
P. Possess a valid California Driver’s license. Must be willing to use own vehicle and valid car insurance for work related travel.
Q. Be willing to use own cell phone to be accessible and available to receive calls in emergency situations with supervisors and relevant colleagues
R. Proficiency in Microsoft Word, Excel, Adobe/PDF, Google Suite (i.e., Google Calendar, Docs, Drive), and Electronic Health Records

DESCRIPTION OF JOB RESPONSIBILITIES AND DUTIES
Provide comprehensive Substance Use, care coordination (aka case management), and rehabilitation services to adults with moderate to severe psychiatric disabilities. This is an exciting position within a multidisciplinary team. The Substance Use Specialist will assist the team in development of treatment plans that address Substance Use recovery goals. The Substance Use Specialist will provide Substance Use counseling, care coordination, linkage, housing support, and other services to individuals in their natural settings (community, homes, shelter, park, hospital, etc.) and at the office. Our clients experience psychiatric hospitalizations, housing instability, justice involvement, severe and acute mental
illness, as well as homelessness. This program will take a do-what-it-takes attitude to support those we serve to find hope, stability, and recovery.


KNOWLEDGE AND SKILLS
A. Consult with staff on Substance Use and program matters as the need arises.
B. Strong understanding of alcohol and other drug addiction and recovery.
C. Strong understanding of resiliency, recovery and wellness approaches.
D. Competence in basic interpersonal skills (e.g., attending & listening, exploring thoughts and feelings, curious questioning)
E. Knowledge about educational, judicial, health care, housing, shelters, emergency, and other social service systems in community preferred
F. Knowledge of clinical documentation and billing standards


DIRECT SERVICES
A. Provide and coordinate Substance Use assessment, Substance Use treatment planning and services delivery tailored to the needs of individual consumers.
B. Provide comprehensive services in service program(s), included, but not limited to, evaluation, rehabilitation, case management, collateral, crisis intervention, and consultation.
C. Assist a consumer with specific, unique Substance Use related case management needs.
D. Assist in developing and implementing treatment/service plans.
E. Incorporate AODA Treatment Design and knowledge in staff treatment planning meetings and facilitate weekly AODA weekly psychotherapy group
F. Conduct timely reviews, updates, revisions as required, and as needed for each consumer, taking into account cultural and personal attributes and wishes.
G. Maintain contact with consumers to a degree sufficient to monitor and assess progress with Substance abuse recovery.
H. Work closely with counselors and prescribers on the team to assure that medication needs and issues are integrated into recovery plans.
I. Arrange for provision of crisis management services as clinically necessary.
J. Complete evaluation tools, initial screening, and gather information for assessment
K. Schedule and attend regular appointments with clients in Hume Center offices, homes, and community settings to assist clients progress in successfully accomplishing goals.
L. Make contact with, and include as appropriate, parent, family, caseworkers and other collateral contracts.
M. Staff the crisis phone on a rotating basis with other staff
N. Perform all evaluations, interventions, and outcome measurements on-time, including data entry of the results as needed.


OTHER
A. Support and cover for other team members as necessary.
B. Meet billable hours set by The Hume Center.
C. Attend weekly staff meetings, trainings, group and individual supervision.
D. Participate in quality assurance processes within the program and department and ensure all documentation meets all relevant internal and external standards.
E. Participate in program development, implementation and evaluation.
F. Community outreach.
G. Meet with Supervisor on a weekly basis for individual supervision
H. Demonstrate an understanding of The Hume Center’s Philosophy, Organizational Culture, and Organizational Channels of Communication through practicing an enabling model of working together
I. Attend other required meetings
J. Fulfill any additional duties specified by Supervisor, Department Head, and/or President/CEO
