JOB SUMMARY
As a Care Coordinator (CC) specializing in Domestic Violence and victims of crime referrals, the CC plays a crucial role in ensuring timely and comprehensive support for individuals impacted by these sensitive issues. Collaborating closely with our Domestic Violence team, the CC will be responsible for efficiently addressing all referrals related to Domestic Violence and victims of crime. The primary focus will be on coordinating services and resources to meet the unique needs of each individual, guiding them through the process with empathy and professionalism. The CC will ensure that all internal and external referrals are connected to a case manager.
This position is grant-funded. The grant is scheduled to expire on December 31, 2024. The grant may or may not be funded again after that date. Your employment beyond the grant expiration date is not guaranteed. Should the grant not be funded, you are eligible and encouraged to apply for another position within the agency.

Qualifications/Licensure:
- Employees are responsible for maintaining individual certifications.
- CA Driver’s License
- Bilingual English/Spanish
- Self-Motivated
- Must have experience working with diverse populations.
- Must have experience working with victims of crime
- Familiar with MS word, Excel, and Power point
- Maintains personal appearance when interacting with patients, staff, and physicians.
- 40-Hour Domestic Violence Certification Training (Must obtain within 60 days of hiring)
ESSENTIAL DUTIES AND RESPONSIBILITIES
Performs a combination, but not necessarily all, of the following duties:
- Monitor scheduling of clients.
- Monitoring victim advocate's caseloads.
- Address internal and external referrals.
- Attend trainings related to victim advocacy.
- Submit monthly reports related to referrals.
- Create and maintain program files and records.
- Attend all program-related meetings.
- Maintain outreach activity calendars and logs according to program standards.
- Attend appropriate community resource meetings and trainings, as assigned.
- Work in collaboration with other departments when required.
- Attend meetings, trainings, and other work-related events as needed.
- Other duties may be assigned or may be modified as business needs dictate.
- Conduct client intakes.
- Provide brief, focused intervention for clients in crisis.
- Knowledgeable in community resources.
- Link clients to outside organizations as needed.
- Respond to all correspondence related to referrals.
- Function as a member of a multi-interdisciplinary team.
