We are adding a Home Care Service Coordinator to our team! We are looking for organized bilingual (English/Spanish) customer service professionals. Our Coordinators are a liaison between our Home Health Aides, Patient/Client and the RN-Home Care Supervisor. As a Service Coordinator you will:
- Identify and assign staff to meet client needs.
- HHA Exchange system Knowledgeable.
- Communicate with RN-Home Care Supervisor regarding all new clients, changes in staff assignments and client needs
- Provide general information to the public regarding home care services
- Collect initial intake information from clients
- Ensure employee files are complete and up to date with all required documentation
- Be available on a rotating schedule for On Call evening and weekend service
- Insures that all Home Attendants are oriented to their clients’ respective needs and plansof care. Calls the client to verify the arrival of the Home Attendant and completes and places a progress note in the computerized Client Profile.Case management and follows up on home care worker and client need.Insures continuous client coverage in accordance with authorized hours.Inputs all changes in service hour authorizations into computer system immediately upon receipt of such authorizations.Insures that client records are up-to-date and complete and thatthey are continually maintained in all areas of compliance contract.Logs time sheets, processes timesheet for payroll and assists in resolving payroll problems. Weekly and weekends.Answers calls from home attendants relating to payroll and timesheet.
