Summary:
The Care Coordinator provides clinical case management and care coordination services in support of client engagement, recovery and reintegration into the larger community. The Care Coordinatoris responsible for the coordination of case management services and facilitation of client direct care services as well as some collaboration with referral sources. The Care Coordinator will also collaborate with the multi-disciplinary team to ensure inclusion of clinical case management services in each individual’s service plan and ongoing communication of client status and progress. The Care Coordinator will comply with contractual guidelines of all regulatory bodies as well as guidelines set forth by the 1115 Waiver.
Essential Duties and Responsibilities:
· Provide clinical case management and care coordination services in support of program engagement, recovery and reintegration into the community through coordinating community care needs, such as medical, dental, housing referrals, assistance with basic needs applications, and other external services and benefits throughout the episode of care.
· Facilitate non-clinical Intensive Outpatient groups as assigned.
· Collaborate with community sources and referral sources by providing communication, including but not limited to: court letters, attendance and compliance reporting, correspondence and telephone communication to coordinate care and provide status updates.
· Coordinate client appointments and work to apply/provide client with Veyo or Bus passes if appropriate.
· Review drug screen results, ensure documentation in the clinical record is complete and communicated with the treatment team.
· Work with the client and the treatment team to coordinate clinical discharge plan.
· Input all required documentation in Connecticut Renaissance’s EHR accurately and timely, including DDaPs for all IOP and OP clients.
- Communicate with treatment team on the status of each client’s progress within the program.
- Facilitate completion of satisfaction surveys for clinical programming.
- Participate in all required internal and external meetings and supervisions.
- Complete all required internal and external trainings to enhance knowledge and skill level. Staffing
- Contribute to meeting or exceeding agency outcomes and performance measures.
· May perform other duties assigned by supervisor.
Required Knowledge, Skills, Abilities and Other Characteristics:
Knowledge of:
· Electronic Healthcare Record systems.
· HIPAA regulations and other regulatory laws
· General office software, particularly Microsoft Office Suite.
Ability to:
· Interact effectively with clients, staff, referral sources and community collaterals.
· Analyze and solve problems effectively and efficiently; work and make decisions independently.
· Communicate effectively both verbally and in writing.
· Manage priorities to meet deadlines and demonstrate multi-tasking skills.
· Assimilate to new information and technology.
Education and Experience:
· Bachelor’s Degree
· Experience with Behavioral Health and Clinical Case Management preferred
Other:
- Must be dependable and reliable; detailed oriented; and able to interface easily with clients, staff, and referral sources.
· Must successfully complete the introductory probationary employment period.
· Must have a valid CT driver’s license.
