GENERAL STATEMENT OF JOB
The Care Coordination Director is responsible for design, development, and oversight of Vaya’s Care Coordination Team. This team provides Care Coordination interventions for Vaya’s Medicaid plan members and performs outreach and engagement functions for Vaya’s Tailored Care Management program. The Director works closely with leadership in Population Health Departments to ensure members experience seamless, high-quality care along the Care Coordination/care management continuum. This leader integrates Vaya’s strategic vision into the culture of the Care Coordination Team and translates organizational goals into operational strategy.
ESSENTIAL JOB FUNCTIONS
Direction and Oversight:
- Direction and oversight of Care Coordination. In collaboration with other leaders contribute to the components of Vaya’s population health management strategy and programs.
- Develop and implement the philosophies, policies, procedures, and goals for Care Coordination.
- Prepare and evaluate operational reports
- Ensure compliance with applicable regulatory, contract and accreditation requirements.
- Select, develop, motivate, train, and retain competent, engaged staff. Provide opportunities for professional development. Reward and recognize accomplishments and successes. Manage performance improvement, discipline processes and terminations, as appropriate.
- Devise ways to optimize procedures that employees are expected to follow during and between calls and member contacts
- Reviews trends for phone volume, staffing levels, and member satisfaction to determine where process improvements may be made
Collaboration and Leadership:
- Set an example for staff related to culture and engagement. Uphold the organization's core values and ensure staff are acting in a manner consistent with them.
- Collaborate with internal/external leadership and key stakeholders to develop and maintain strong relationships with Care Coordination delivery systems to achieve population health objectives and goals.
- Help create and maintain a positive department culture through the participation in, and creation of culture initiatives

QUALIFICATIONS & EDUCATION REQUIREMENTS
Master’s degree in healthcare, behavioral health, and/or business required. Must have a minimum of 5 years of supervisory experience in a management role in the healthcare field.
PHYSICAL REQUIREMENTS
- Close visual acuity to perform activities such as preparation and analysis of documents; viewing a computer terminal; and extensive reading.
- Physical activity in this position includes crouching, reaching, walking, talking, hearing and repetitive motion of hands, wrists and fingers.
- Sedentary work with lifting requirements up to 10 pounds, sitting for extended periods of time.
- Mental concentration is required in all aspects of work.

SALARY: Depending on qualifications & experience of candidate. This position is exempt and is not eligible for overtime compensation.
