Summary of Responsibilities
The Cardiovascular Health Program Manager supports the Cardiovascular Health Program and works collaboratively to achieve reductions in the burden of chronic disease and cardiovascular disease in Georgia. Under broad supervision, the incumbent is responsible for assessing, planning and/or coordinating training sessions for the Cardiovascular Health Program. The Program Manager is responsible for providing ongoing, technical, and training support to Public Health Districts and Public Health partners, specifically around hypertension control and management and cardiovascular disease prevention. The incumbent will work collaboratively with the Cardiovascular Health Team to support the promotion of evidence-based prevention programming and best and promising practices, and will ensures stakeholder engagement in carrying out program activities The Cardiovascular Health Program Manager Reports to the Cardiovascular Health Team Lead.
Management
- Requires independent program management and the ability to make decisions to improve program and service efficiencies.
- Uses project management tools and techniques to manage projects and activities related to Office programs and projects.
- Collaborates effectively with staff above, below, and across the organizational structure and Department to ensure the successful execution of projects and ideas.
- Seeks input and engages others to create a positive and team-based environment that contributes to overall chronic disease prevention efforts for the State of Georgia.
- Manages interactions with prevention and health promotion partners, as directed, to achieve outcomes and goals.
Programmatic
- Builds and maintains key external relationships with a wide range of cardiovascular/heart health and healthcare stakeholders, including managed care organizations and health systems.
- Builds and manages a coordinated network of multidisciplinary partnerships that address identified barriers to social services and support needs within populations at the highest risk of CVD.
- Uses creative and strategic thinking skills to find new and innovative ways to engage with stakeholders while furthering DPH's Cardiovascular Health strategies.
- Participates as a hypertension and/or high blood pressure/cholesterol content expert in interdisciplinary/interdepartmental meetings as needed.
- Oversees the planning, implementation, and evaluation of interventions to support the use of team-based care in hypertension and cholesterol management in clinical settings.
- Oversees the planning, implementation, and evaluation of interventions to support the community-clinical linkages in hypertension and cholesterol management in clinical settings.
- Oversees the planning, implementation, and evaluation of Pharmacy-Related Interventions to manage high blood pressure, high cholesterol, and lifestyle modification.
- Communicates about program development and implementation to a wide range of stakeholders. Tailors presentations/engagement activities for various audiences such as healthcare/public health organizations, business groups/employers, academic institutions, legislators, and major stakeholders.
- Applies public health knowledge to develop analyses of programmatic and intervention needs in communities related to cardiovascular disease prevention and management.
- Creates proposals for new programs and approaches to program implementation. Engages partners in the development of those proposals and identification of new programmatic goals.
- As directed, represents the Section and Department in meetings with external stakeholders.
In collaboration with the Cardiovascular Health Team:
- Communicates about the program, planning, and policy implementation to a wide range of stakeholders. Prepares, analyzes, and distributes program reports, graphs, charts, and other data.
- Monitors and evaluates DPH Cardiovascular Health programs to determine their effectiveness, level and quality of customer services, and the need for improvements, and initiates appropriate actions.
- Assists in assessing needs, designing, and implementing new, innovative Health Education course offerings, including online, app-based and other technology-based course formats.
- Participates in internal & external marketing of hypertension and high cholesterol prevention and management programs to clinicians, staff, and the community.
- Participates in a coordinated network of multidisciplinary partnerships that address identified barriers to social services.
- Promotes the adoption of evidence-based strategies, including blood pressure self-monitoring, to manage and control hypertension in clinical and community settings.
- Promotes the adoption of systems to refer adults with hypertension and high cholesterol to community programs and resources.
- Through collaborations with interested parties inside and outside the agency, helps develop local and statewide systems and tools that will enhance cardiovascular disease prevention and management. Increases the availability of systems, tools, and services to improve hypertension control and cholesterol management in Georgia.
- Provides technical assistance, education, and training to district and county health departments and other Cardiovascular Health partners. Creates and provides training opportunities for hypertension, high cholesterol and other risk factors related to chronic disease outcomes.
- Participates in learning collaborative with multi-sector partners and participants who commit to improving perinatal birth outcomes through service linkages, information sharing, and systems change.
OR
Bachelor's degree in a related field from an accredited college or university AND Five years of experience performing work related to area of assignment
OR
One year of experience at the lower level Healthcare Program Consultant 2 (HCP091) or position equivalent.
Preferred Qualifications
- Master of Public Health degree or another applicable field of study (MPH or M.ED Instructional Design)
- Experience as a quality coordinator for a chronic disease management program
- Experience in hypertension, chronic disease and work in healthcare settings is highly desirable.
- Experience with Motivational Interviewing and facilitating behavior change
The use of wellness breaks during the workday is authorized to support this philosophy and assist employees in meeting their wellness goals. A maximum of 30 minutes in a given workday may be used to engage in wellness activities, generally in the form of two 15-minute breaks or one 30-minute break.
