Summary:

The Cardiac Rehab & Wellness RN collaborates in a team environment to coordinate patient care for the Phase 2, Phase 3 and Intensive Cardiac Rehab Services. This includes nursing assessment, problem identification, planning, reporting of complex clinical data to the appropriate Cardiologist and implementation of the patient’s plan of care as directed by the provider. In addition, this person delivers and coordinates patient wellness education, risk factor management with a multi-disciplinary Team.

Responsibilities:

• Completes new patient intakes for all Cardiac Rehab and Wellness services.
• Leads exercise classes with the Exercise Physiologist(s) for the Phase II/Phase III Cardiac Rehab, Lifestyle Management programs; ensures safe use of exercise equipment and body mechanics.
• Monitors telemetry during exercise and triages patients’ physical symptoms/arrhythmias per protocol.
• Ensures the Individualized Treatment Plan are updated per required regulations.
• Educates patients about their cardiovascular condition and explains treatment options as directed by their provider.
• Participates in Transition of Care Management to ensure discharge medications and instructions are understood by patient.
• Initiates and maintains personal professional development as required for continuing education.
• Maintains patient confidentiality per HIPPA.
• Other duties as assigned.

Qualifications:

Education:

BSN degree highly preferred

Experience:

• Minimum of two-year cardiovascular clinic experience and/or hospital nursing experience.
• Two years’ experience in a critical care or cardiac inpatient unit preferred
• Two years’ experience with telemetry monitoring required.

Licensure:

• Current Registered Nurse California license.
• Maintains BLS and ACLS certification
