Seeking dynamic Exercise Physiologist or RN to join the Flexible Workforce (FWF) team as a Cardiac Rehab Therapist in support of Pulmonary and Cardiac rehabilitation services. In collaboration with patients and families, the Cardiac Rehab Therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.
- Position will primarily provide coverage for MHF Lakes, St. Johns and Woodwinds campuses.
- As needed, FWF team members may cover at other MHF campuses across the Twin Cities metro.
- Schedule will be 8-hours per day, Tuesday through Friday with a flexible start/stop time.

M Health Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. ResponsibilitiesThe Cardiac Rehab Therapist partners with the patient and family while performing the Evaluation/Re-evaluation
Gathers pertinent data, performs examinations, and determines treatment diagnosis
Develops an individualized plan of care through collaboration with the patient and family
Analyzes data to identify needs and potential risks and determines prognosis for recovery
Establishes individualized goals with patient and/or caregiver
Individualizes plan of care considering developmental level and cultural differences
Implements and modifies plan of care based on reassessment and patient response
Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care
Establishes appropriate discharge plan
Partners with patients and families to provide skilled interventionImplements plan of care
Alters treatment/plan of care to reflect change in patient status and response to treatment
Assesses patient and/or caregiver learning needs and provides appropriate education
Documentation/BillingFollows professional practice standards and uses acceptable medical terminology
Completes documentation in a timely manner per department standards

QualificationsRequired License/Certification/Registration
- Basic Life Support prior to end of orientation
- Advanced Cardiac Life Support within 6 months of hire
Current Minnesota licensure as a:
- Physical Therapist, or
- Registered Nurse or
- Bachelor's Degree in Exercise Related field (Exercise Science or Exercise Physiology
Preferred Education, Experience, and Certification
- Master's degree in Nursing or Physical Therapy, or an Exercise Related field (i.e. Exercise Science, Exercise physiology)
- 1+ years' experience
- Certified Exercise Specialist (CES)
- Registered Clinical Exercise Physiologist (RCEP)
- Certified Clinical Research Professional (CCRP)
