Cardiac Rehab Specialist I
0.5 FTE (40 hours bi-weekly)
Hours between 6:30 am - 4:30 pm, Monday through Friday, but may vary based on the needs of the department.
Position Overview:
The Cardiac Rehab Specialist provides therapy programs for patients that qualify for Cardiac or Pulmonary Rehab services. This caregiver provides direct patient care including assessment, development of the plan of care in response to providers prescription, treatment, documentation, and patient education to clients of all ages.This individual directs patient participation and progression in accordance with clinical best practice guidelines to restore, reinforce and enhance patient performance in the prevention of disease and its associated risk factors.They facilitate the learning of skills and functions essential for adaptation to disease.The Cardiac Rehab Specialist is accountable to productivity regarding shared patient volumes and personal and professional growth.They should possess the skills to be adaptable to a dynamic team environment and open to the idea of developing, promoting, and performing new programs to facilitate rehabilitation.Programs may include new covered diagnosis, new equipment available for clients or new education methods.This individual is also responsible for student supervision.
Essential Duties & Responsibilities:

  • Performs direct patient care and evidence-based practice in a safe, ethical, and legal manner.Develops and revises a plan of care based on the initial assessment, patient history, diagnosis, provider orders, protocol, risk stratification, expected outcomes, and patient needs and goals.
  • Evaluates patient response to exercise, measures change and development, shares findings, and makes appropriate recommendations and referrals.
  • Directs patient participation, progression and discharge based on the plan of care and "incident-to" recommendations from the referring provider(s) in accordance with clinical best practice guidelines to restore, reinforce, and enhance patient performance in the prevention of disease and its associated risk factors.
  • Documents all skilled and medically necessary care thoroughly, timely, and accurately in accordance with departmental policy.Documentation, charges, and care must comply with regulatory requirements as well as departmental policy.
  • Routinely communicates patient response to exercise to referring provider and alerts providers of any negative or unexpected response to exercise.May assist with acquiring physician orders and review of monthly statistics.Maintains patient and organizational confidentiality at all times.
  • Actively participates in activities necessary to maintain departments national certification
  • Schedules, instructs and connects patients for Holter/Event monitoring with computerized report to physician as required.
  • Prepares and performs electro cardiology studies in cooperation with providers together with Winona Health imaging staff, processing documentation for ordering and attending provider as required.
  • Performs duties in accordance with departmental productivity standards.Independent in using time wisely.
  • Promotes service excellence with patient scheduling and accommodation with appointments.
  • Responds positively to changes in the staffing schedule and facility to accommodate patient needs within hours of operation.
  • Uses available time to develop programs, complete projects, or assist the department as needed.Promotes the appropriate utilization of resources.
  • Collaborates and consults with other health care professionals and shares gained knowledge.
  • Serves in a mentorship capacity to students.Serves on departmental or organizational committees as appropriate.Delivers a high level of customer service to all internal and external customers.
  • Fosters and promotes the cardiopulmonary rehab profession; attends professional continuing education courses and hospital in-services, required for professional certification.
  • Strives to master the skills necessary to exceed the expectations of the people we serve.
  • Works effectively as a member of the rehab team and Winona Health team through collaborative partnerships to promote cardiac and pulmonary rehab and foster effective communication and departmental planning.
  • Participates in performance improvement and completes all required in-service programs to ensure standards are met and skills are kept current.
  • Participates in focus board and metric tracking for quality improvement.
  • Other duties and responsibilities as assigned based on facility need and requirements.

Essential Skills and Experience:
Bachelor's degree in Exercise Science, Cardiac Rehab, or related field of study. BLS.ACLS. Internship in cardiopulmonary rehabilitation is required. Two years clinical experience in cardiopulmonary rehabilitation and experience in stress testing preferred. Basic computer skills including Word, Outlook, and Excel.Patient information database (Cerner, EMR, American Data). Must possess good time management skills, good communication skills, the ability to adapt to changing situations, a positive attitude, excellent customer service skills, the ability to problem solve and suggest solutions, and the ability to function successfully in a team environment. Must be able to obtain ACSM clinical exercise physiologist certification within 18 months of hire date.
Physical Demands:
Light-Medium Work
Work activities that require lifting frequently with 10#, occasionally with 15-20# and rarely 25#. Work activities that require push/pull force of 20-30# on frequent basis and rarely up to 40#. Light -Medium work generally is defined as all the work activities in the Light category of work except there is a higher push/pull force on a frequent basis and higher weight for lifting activities on a frequent basis compared to the Light work category.
Physical Requirements:
  • May lift and carry supplies that weigh between 10-15# and max of 20#-35#
  • May push/pull carts on frequent basis with force of 20# to 40# (included all floor care machines, moving wheelchairs into the van) up to 70# rarely.
  • May Push/pull wheelchairs more frequently for longer distances.
  • May assume reaching ranges frequently between vertical heights of 20-36" in either sitting or standing position to complete work activities. Reaching to retrieve items from storage may require occasional reaching at vertical heights of 18-20" and 40-72".
  • May assume frequent reaching to the front and side and occasional reaching above shoulder height while working in either sitting or standing positions.
  • May require light to moderate grip or pinch force to complete work activities.

Required Work Schedule:
Generally working hours are 6:30 am to 4:30 pm Monday through Friday, but may vary depending on the census and needs of the department.
