Bridge Patient Care Coordinator

Aegis Health Care is currently seeking a compassionate Bridge Care Coordinator that has a passion for providing outstanding services to the elderly in Senior Living Communities and in-home health settings. This is a part-time position serving our patients in the Anchorage, AK area.
Each Bridge Care Coordinator is an important part of the Aegis Health Care team. Aegis specializes in providing care for hospice and home health patients. Bridge is a relationship-based care model, one that benefits patients when they need an added layer of support. Job Responsibilities:
- Perform scheduled in-home patient visits
- Establish a trusting and supporting relationship with patients and their families
- Educate families on the Aegis Health Care continuum
- Connect patients with outside resources (e.g., community resources, social services)
- Attend regular staff trainings and other meetings as requested
- Capture and communicate relevant information about the patients' health by accurately completing standard patient visit checklist
- Maintain proper records of visits
- Accept and perform other related duties and responsibilities as required
Skills and Qualifications:
- Experience in patient centered care environment
- Clinical experience is a plus, such as certified caregiver or CNA
- Ability to articulate Aegis Health Care mission and values
- Interpersonal communication skills with exceptional listening abilities
- Compassionate and caring attitude
- Ability to communicate effectively both verbally and in writing
- Valid Driver's license, clean driving record, and reliable automobile
- Proof of current automobile insurance
- Criminal background check clearance/fingerprint clearance
- Current CPR certification
- Negative TB test
