Position Summary

The Building Resilience, Ease & Acceptance Through Healing Experiences (BREATHE) program responds to patients and clients of WellSpace Health who indicate an elevated risk to their mental, behavioral, social, and biological health on an Adverse Childhood Experience (ACEs) screening or present with symptoms of toxic or traumatic stress. The program supports individuals in mitigating the impacts of their experiences through therapy, health education, peer support, and access to buffering services to support individual healing and reduce the risk of intergenerational transmission of ACEs and toxic stress.  Working closely with the Director and the Program Manager, the BREATHE Intake Clinician liaises with the WellSpace Clinical Programs staff and partner services, including WellSpace Health Primary Care, Pediatrics, Behavioral Health, and other community services.  The Intake Clinician receives and quickly responds to referrals and provides a basic individualized needs assessment to support connection to program, organizational, and crisis support services as needed.

The Intake Clinician is the first team member to engage patients and maintain contact with them until services are assigned to the appropriate team member by the Director or designee, and supervises para-professional, student intern, and case management staff. With the team, the Intake Clinician will support the early development of the BREATHE program pilot stage while scaling it to respond to referrals from more than 30 healthcare centers and behavioral programs. The team will provide services in multiple healthcare centers within three counties, beginning with adults and expanding to services for families and children. This is an on-site position with the possibility of a hybrid schedule after 90 days.

 

Required Qualifications

  • Master’s Degree in Social Work, Psychology, Marriage and Family Therapy, or a similar field and registration or Licensure with the California Board of Behavioral Sciences (BBS) is required. Licensure is preferred.
  • A minimum of two years of direct service post-graduation from a master’s program with the ability to supervise student interns is preferred.
  • A minimum of one year of general staff supervision is preferred.
  • Training and knowledge of the impact of ACEs and building mitigating and protective factors is preferred.
  • Training and experience in understanding toxic stress and trauma is preferred.
  • Provide and model the standards of Trauma-Informed Care and Just Culture.
  • Knowledge and skill in working with a very diverse population is required.
  • Ability to practice and model cultural humility.
  • Ability to remain calm, maintain professional boundaries, and build a strong therapeutic alliance while in stressful and highly emotional situations.
  • Ability and willingness to practice and model self-care techniques.

  • Assist and support the Center/Department/Program to meet standards of High Reliability.
  • Ability to work independently within the program structure and collaborate with the team, supervisors, and other services provided by WellSpace Health.
  • Support the organization and program to meet standards of High Reliability.
  • Valid California Driver’s License with an insurable driving record.
  • Ability to maintain a confidential workspace if approved to work remotely.

Essential Responsibilities 

  • Supervise BREATHE program staff of Case Managers and Student Interns. 
  • Oversee all service and administrative functions of the BREATHE support services, including case management, health and psycho-education groups, and peer support services.
  • Provide weekly supervision for administrative and service-focused duties of assigned staff members.
  • Supervise all referrals to the program, document, and bill for services using the electronic health records system.
  • Collaborate with program management and leadership to recruit, train, and coach program staff.
  • Proctor graduate and undergraduate student interns (bachelor and master level) within organizational agreements and assignments.
  • Coordinate all aspects of programmatic groups and activities.
  • Record, respond to, and track all referrals to the program, ensuring closed-loop communication with patients, clients, and the referring party.
  • Provide health education about ACEs, a brief assessment of patient interests and needs, offer services and support them through connecting to resources.
  • Collaborate with the program team and partners to meet client and programmatic goals.
  • Collaborate with program management to ensure all referred parties are connected to the appropriate services and support the transition of the patient to the therapist as assigned by the Director of the designee.
  • Provide high-quality, trauma-informed, culturally competent services to adults, youth, and children.
  • Utilize interpretation services as needed.
  • Participate as a member of a multi-disciplinary team (MDT) of service providers.
  • Maintain client and patient records per organizational policy.
  • Proctor graduate and undergraduate student interns within organizational agreements and assignments.
  • BBS Associate clinicians may receive clinical supervision from BBS Licensed Clinicians and must adhere to BBS standards for supervision.
  • Perform risk assessment and crisis management, including evaluation and referral to Emergency Services for patients who are suicidal, homicidal, or experiencing intimate partner violence and child maltreatment.
  • May provide support group services, short-term case management and clinical services as needed.
  • Will partner with Case Managers to support clients in practicing coping, regulation, and resiliency-building skills.
  • Make recommendations on outcomes and program evaluation. Assist with related data collection. Assist with related projects and initiatives. Assist with Utilization Review activities.
  • Attend weekly individual and group supervision and staff meetings.
  • Travel within the WellSpace Health service area as assigned to complete the position's duties, approximately 15%.
  • Other duties as assigned.

Physical Demands and Work Environment

The work environment is characteristic of a medical clinic environment.  The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands and arms to reach.  The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus.   The employee must also possess hearing and speech to communicate in person and over the phone.  The noise level in the work environment is usually quiet to moderate.

The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care.  The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
