Position Description:

We are looking for a Blended Case Manager to join our Team!

Nulton Diagnostic & Treatment Center is accepting applications for a Blended Case Manager in Somerset County. The successful candidate will have knowledge of mental health services for adults and children/adolescents. This full-time position will assist individuals in accessing, monitoring, linking and coordinating mental health treatment while providing needed advocacy and support.

Education, Experience, Skills, Training:

Minimum Education:

A. Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or,

B. Registered nurse; or

C. High school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years’ experience in direct contact with mental health consumers; or

D. High school diploma and five years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989.

The successful applicant must be able to obtain Act 33, Act 34 and FBI clearances.

This is a full-time, salaried position which includes a competitive pay, 401(k) Retirement Plan with matching contributions, specialized training, and medical, dental and vision benefits, as well as, paid vacation and paid holidays. This position also offers Incentive-Based Bonuses.

• Max pay possible for 1st year is $50,000 (this includes salary, on-call pay, gas and cell phone stipends, productivity bonus and sign on bonus).
• Merit based raises, at 1, 2 and 3 year anniversaries.
• 401k with a 4% match (eligible to contribute after 1 year)

Urgent Hire $4,500 Sign-On Bonus! $4,500 Sign-On Bonus for anyone with 12 months or more of BCM experience! $3,000 bonus for all others.
