Bilingual Long Term Care Manager - Hybrid
HealthCare Support is currently seeking an experienced Bilingual Long Term Care Manager - Hybrid to fill an opening with a Fortune 100 Managed Care Company in Orange and Seminole Counties FL. This is a 12 month contracted assignment.
**This is a Field/Remote role open to those who live in Seminole and Orange County FL only!**
**Must own reliable vehicle and have a clean driving record. Mileage is reimbursed.**
The ideal candidate will have the following requirements for the Bilingual Long Term Care Manager - Hybrid role:
- Contractors will work from home but will be visiting members in facilities or their homes. 80% is member facing. Must live in Region 7 - Seminole/Orange county area but able to travel to surrounding Region 7 counties as needed( (Osceola, Brevard)
- Must be bilingual in Spanish
- Bachelor’s degree
- OR Registered Nurse License and 2+ years of care management experience
- OR LPN with 4+ years of case management exp
- 2+ years of case management experience
- Experience with electronic medical health records
- Must have valid FL Drivers License
Assignment details for the Bilingual Long Term Care Manager - Hybrid
- Schedule: Monday - Friday 8am-5pm
- Start date is approximately Jan 8th 2024
- This is a 12 month contract
Travel: Monthly and quarterly member contact and will include 80% travel - Will receive IRS mileage rate which is currently 65.5 cents per mile
- they try to keep the travel radius within 15-30 miles of where the candidate lives
- most likely will have a mixed case load of seeing members in their homes, and members at nursing facilities ASL/LTC
- Candidate is able to pick when they do their member visits each week but they must see their members at least 1x every 3 months so on average they will do about 3 visits each week but will depend on their case load
- On average each visit could be 45 minutes-1 hour long, normally the new member orientation could go over an hour but that will only be completed with new members
Day to Day Details for Bilingual Long Term Care Manager – Hybrid
- Managing a case load for healthcare members with Long Term Care needs.
- Monthly and quarterly member contact and will include 80% travel. Remote role. Will require a drivers license.
- Complete assessments with members, caregivers, or providers to obtain information regarding client status, support system, and need for services for care plan development’
- Authorize and coordinate referral for services.
