Our collaborative team is small and mighty. Our team is made up of a physician medical director who is board certified by the American Board of Family Medicine and the American Board of Preventative Medicine in Addiction Medicine, a nurse practitioner who is dual board-certified as a Family Nurse Practitioner (FNP) and in Psychiatric-Mental Health (PMHNP), a doctoral level Behavioral Health coordinator (PhD, LMFT, LAC), two additional Behavioral Health providers, and three medical assistants. The culture of collaboration and teamwork is something we are very proud of.
Your work would include Behavioral Health assessment and intervention, resource connection, addressing barriers to care and treatment, treatment adherence and monitoring among others in our Provider Addiction Clinic.
Qualifications:
  • Provide brief Behavioral Health support to persons living with addiction
  • Monitor adherence to medical and Behavioral Health treatment
  • Connect patients with community resources for biopsychosocial, financial, and other needs
  • Communicate / collaborate with medical, interdisciplinary team staff, patient / family, insurance companies, social agencies
  • Timely documentation of all services within the clinic's electronic medical record
  • Participation in patient care conferences as appropriate
  • Assess coping mechanisms and current functioning in following areas: life skills, social skills identity, interpersonal, family and group dynamics
  • Plan and develop group and individual treatments as a member of a multidisciplinary team
  • Implement group, individual and family treatments ensuring a safe, comfortable therapeutic environment
  • Ensure the follow-through of treatment goals concerning individual group therapy
  • Document patients' progress and response to treatment in a timely manner
  • Provide brief intervention using motivational interviewing techniques to individuals with identified needs who are living with chronic illnesses.

Required
  • Masters degree in a related field
  • LMFT, LAC, LCSW and/or LPC
  • Three years of relevant work experience

Preferred
  • Licensed Addiction Counselor
  • Five years of relevant work experience

Physical Requirements:
Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies. Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations. Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc. Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items. Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc). May be expected to stand in a stationary position for an extended period of time.
