New Horizons Healthcare (NHH) is seeking a full-time Behavioral Health Case Manager. The position is responsible for providing case management services to patients with a mental health diagnosis by assessing needs; providing resources; coordinating services within the clinic and in the community to meet patient needs; facilitating referrals; developing, and implementing, monitoring and evaluating a plan of care, with the overall goal to ensure patient access to services appropriate to their health needs and to optimize the patient’s health.

New Horizons Healthcare is a federally qualified health center (FQHC) in Roanoke, Virginia that provides comprehensive primary care, Behavioral Health, dental care, and pharmacy services for almost 10,000 patients annually. COMPETENCIES REQUIRED
The following competencies are required for this position:
Communication: This position requires a collaborator with excellent written and verbal communication skills, along with the ability to communicate with patients, families, Behavioral Health and medical staff, management, and community partners to promote and advocate for patients. Documents, within required time limits, all service contacts and maintains the patient records in accordance with agency and regulatory standards and requirements.
Teamwork: This position requires a professional willing to accept and share constructive feedback to benefit the patient, organization, and the community. Willingness and flexibility to adapt schedule as needed to provide necessary support for patients, families and community partners. Maintains professional relationships and works effectively within the clinic and throughout the community to promote optimal patient outcomes.
Problem solving: Ability to manage time and resources; ability to prioritize patient needs and utilize resources to effect optimal patient outcomes and remove barriers; to provide patients, families and community members quality information and guidance regarding quality Behavioral Health service.
Self-Management: Demonstrates effective time management, critical thinking, and prioritization of multiple priorities; responsible for participation in ongoing quality improvement; assessment of progress against case management plan and goals and modify as needed.
Planning and organizing: Complete comprehensive assessments and identify New Horizons patients for Behavioral Health services utilizing screening tools; participate in “warm hand-offs” with clinical staff; develop individualized plan of care with input from member/family that includes long and short term goals; collaborate with PCP and other members of the health care team in plan of care; scheduling follow up and communications with patients to monitor the quality of care to determine if the plan of care is being achieved; coordinate and modify resources and services needed to evaluate the effectiveness of the plan of care.
Technology: Utilize various technologies to provide patient care and improve outcomes; utilization of data to support and measure quality metrics; report development; computer literate with working knowledge of Microsoft Office, EMR, and various other computer software programs.
Learning: Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities. Participation in organizational staff development opportunities, and staff or committee meetings as directed.
QUALIFICATIONS/KNOWLEDGE/EXPERIENCE
  • Qualifications – bachelor’s degree in psychology or social work preferred, QMHP-A preferred.
  • Knowledge, Skills & Experience (Essential): Candidate must be collaborative, with excellent communication skills, and a strong orientation in customer service. Interact effectively with patients, staff, and community members from diverse backgrounds.
  • Minimum of three (3) years’ experience providing case management services.
  • Experience working with needy, disadvantaged, or high-risk populations.
OCCUPATIONAL HEALTH AND SAFETY
Work is partially performed (100%) in a normal, pleasant office environment. A portion of work (10-15%) may be performed in community outreach activities. Moderate physical activity is required, lifting up to 25 lbs., standing or walking for more than four (4) hours per day.
