Care Management:Provides direct care management and care coordination to TU students at the University Health Center during regular center hours. Communications with students may be in person, virtual, or via telephone. Coordinates referrals to specialists and/or specialty services as needed and as appropriate. Provides ongoing care in coordination with the rest of the Health Center team. Documents within the electronic health record. Systematically tracks patient follow up, clinical outcomes, and treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications; treatment adherence monitoring; and provides brief behavioral interventions. Consults with the Medical Director and/or Staff Providers on complex cases or as needed. Immunization and TB Screening Compliance Coordination:Collects, analyzes, monitors, and maintains compliance data. Prepares reports reflecting compliance data. Understands, interprets, and disseminates knowledge on immunization compliance and tuberculosis risk screening. Evaluates and tracks immunization compliance. Authorized to place and remove holds and fines on student accounts. Communicates with students regarding their immunization compliance status. Problem solves and responds to students, parents and families regarding immunization questions and concerns. Creates immunization appointments, receives, and sends and maintains emails relating to compliance.Participation on University and Departmental Committees:Participates in various university committees having a mental health or care management focus. Participates in staff meetings and quality assurance activities, quality improvement projects as assigned, and development of clinical protocols and guidelines. Performs other administrative duties as assigned by the Medical Director.Continuing Education:Participates in continuing education to improve knowledge of care management, Behavioral Health topics, and compliance and satisfy certification and licensure requirements.
Qualifications and Skills
Required:
- Bachelor's degreeOne year of Care/Case Management or Social Work experience.An Associate’s Degree in Nursing will be considered if the candidate is a Registered Nurse with three years of relevant work experience. Licensed Clinical Social Worker, LCSW-C with licensure in the State of Maryland (preferred), or Registered Nurse (RN) in the state of Maryland or Licensed Bachelor Social Worker (LBSW) in the state of Maryland.Experience and proficiency in providing care/case management to primary care or psychiatric patients. Proficiency in Microsoft Office Suite including Word, Excel, and PowerPoint.Experience and proficiency in navigating electronic health records.
- Master’s DegreeExperience working in the college health setting. Experience working on a multidisciplinary mental health team. Must be organized and have a strong attention for detail; ability to meet deadlines, prioritize responsibilities, and coordinate a case load .Maintain quality customer service skills; possess excellent interpersonal and coalition-building skills; and have a strong work ethic with the ability to work independently and as part of a team. Must have strong communication skills as well as sound interpersonal skills. Ability to embrace and support TU’s commitment to diversity, equity, and inclusion.
