Counseling/Assessment:
- Provide short term (1-12 times) individual counseling services for DH Patients to improve emotional health and quality of life.
- Provide Brief Solution-Focused interventions and assessments during warm hand off primary care patient visits.
- Demonstrated competence with DSM diagnostic criteria and treatment planning.
- Stay updated on evidenced-based practices, research, and advancements in the field of integrated behavioral health.
- Willing and able to provide occasional crisis intervention and assessment for patients presenting to the Hospital Emergency Room with mental health or substance abuse emergencies.
- Adhere to ethical standards and legal requirements related to patient confidentiality, counseling, and privacy.
- Perform mandated duties as required by CPS, APS and Colorado Mental Health Statute.
- Meet all continuing education requirements for LPC or LCSW licensure.
Collaboration/Teamwork:
- Collaborate with medical and psych providers regarding Behavior Health needs of patients in the DH outpatient clinic setting.
- Coordinate care with external providers, community resources, and support systems to ensure continuity of care.
- Staff challenging patient cases with clinic staff, medical providers, and as needed.
- General understanding of pharmacology regarding commonly used psychotropic medications used in a primary care setting.
- Willing and able to travel between DH outpatients clinics, the hospital, and work in multiple clinic settings as needed.
- Participate in multidisciplinary team meetings and case conferences to provide expertise on behavioral health matters
Documentation/Administrative:
- Complete documentation of patient charts within one business day of meeting with patients.
- Meet all continuing education and procedural requirements for LPC or LCSW licensure.
- Proactively work to meet patient encounter goals and maintain/grow patient panel.
Staff/Education:
- Provide training to clinic staff regarding psychological safety, de-escalation of agitated/angry patients.
- Provide in-service training to DH staff regarding mental health needs, self-care, substance abuse, and ethical issues.
- Perform other duties as assigned by the Behavioral Health Program Director and DH Management.
- Master’s Degree from an accredited School of Social Work or a Master’s Degree in Professional Counseling from a program approved by the Council for Accreditation of Counseling Related Educational Programs (CACREP).
- Must be a LPC or LCSW in good standing in the State of Colorado.
- Minimum of two (2) years of social work experience or Licensed Professional Counselor in a health care setting.
- Must be able to work with clients of all ages and backgrounds, and have clinical diagnostic ability/experience.
- Must have a minimum of one year pediatric/adolescent or other youth counseling experience.
Preferred Experience:
- 1+ year of addiction counseling including working knowledge of alcohol, opioid, and meth addiction, DSM5 criteria, withdrawal symptoms and evidenced-based treatment.
- 1+ year of experience providing integrated medications for Opioid Use Disorder (MOUD) for patients struggling with addiction.
- 1+ year of geriatric counseling and working knowledge of medical/psychological challenges of elderly patients.
- 1+ year of grief counseling experience, specifically supporting patients with chronic or terminal illness.
- 1+ year of supporting patients with chronic pain/physical limitations. Pain Reprocessing Therapy experiences a plus.
- Bilingual Spanish-Speaking.
- Mindfulness-Based Stress Reduction, Mindfulness-Based Relapse Prevention, or other evidenced-based somatic awareness counseling experiences a plus.
- 1+ year EMDR trained/practice.
Other:
- Must maintain an active driver’s license, a personal vehicle in operational order for travel in varying weather conditions at varying times of the day/ night.
- Must maintain automobile/vehicle liability insurance coverage, current proof of insurance and current driver’s license in personnel file at all times.
- Must maintain an operational phone or equivalent means to be contacted except when on leave or vacation.
The Behavior Health Provider (BHP) collaborates with Delta Health medical providers in the assessment, planning, and treatment of social, emotional, and behavioral needs of patients. The Behavior Health Provider works with the patient’s medical provider in diagnosing psychiatric disorders, formulating a treatment plan, providing short-term counseling, and offering preventative services through warm hand offs with medical patients. The Behavior Health Provider contributes significantly to the quality of clinic services as a clinical practitioner, and as a consultant and facilitator in areas of policy development, program planning and evaluation, and staff/culture development and support. The BHP supports patients experiencing emergent needs for placement due to psychosocial factors and assists the hospital emergency department at times by providing evaluation and assessment for patients presenting with mental health or substance emergencies.
