Summary
The Associate Chief Nurse for Patient Flow provides leadership for patient flow and operations that support care delivery across the continuum of care involving complex healthcare issues that influence the VA mission, health care and policy development. The Associate Chief Nurse, Patient Flow is responsible and accountable for the coordination and evaluation of integrated programs that cross service and/or discipline lines.

Duties
The ACN for Patient Flow provides executive leadership and supervision to the Nurse Managers, Assistant Nurse Managers for Bed Control, Emergency Department, Escort Service, and Off Tour Nurse Supervisors. The ACN Patient Flow has substantial and continuing responsibility and accountability for planning, organizing, directing and controlling issues involve in assigned operational areas that primarily impact patient flow across the VA facility, to include the Transfer Center, VA Community Care Home Health, Discharge Planning, Transitions, Utilization Management, and COVID-19 related activities. He/She is accountable for responding to internal and external communications, maintaining knowledge of and communicating with VA and nonVA clinical colleagues.
The ACN Patient Flow is responsible for the administrative functions of his/her programs. He/she has the capacity to function as an administrator, educator, and consultant utilizing nursing theory and leadership theory in collaboration with all services within clinical operations. The Associate Chief Nurse, Patient Flow must establish and maintain effective relationships with all levels of medical center personnel and affiliated centers and relate effectively with customers, families and the community as well as program and service leaders at the local and network levels. Duties and responsibilities include, but not limited to: Leading and organizing delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning. Using advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes. Functioning as an expert in clinical practice and/or areas related to the assigned roles and responsibilities.
Systematically evaluating current practice and formulating outcomes for groups of patients and/or organizational processes within an area of expertise. Guiding, developing, and supporting staff from a leadership perspective. Using professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities.
Central Texas VHCS is in the process of making a cultural change, shifting from a disease-focused approach to a health and wellbeing approach, in order to implement the Whole Health System of Care. This is a VA-wide initiative to provide a comprehensive and systematic approach to improve healthcare delivery at all stages of Veteran care. This entails moving from an episodic, "siloed" approach to an interdisciplinary, holistic approach, emphasizing self-care in the larger context of well-being. All staff in this facility will be part of this process, shifting the focus from the question, "What's the matter with you?" to the question, "What matters to you, and how can we help you live your best life?" This strategy also involves integrating a full range of conventional approaches with Complementary and Integrative Health (CIH) approaches in order to optimize lifelong health and well-being, as defined by the Veteran, rather than narrowly focusing on disease management.

Requirements

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

  • Experience leading/managing multiple programs or services
  • Professional/Shared Governance experience
  • Change/project management experience/certification
  • Evidence-Based Practice experience
  • Systems Redesign and/or Lean/Six Sigma experience/training/certification
  • Experience in a Magnet or Pathway to Excellence facility

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse IV - Master's degree in nursing or related field with BSN and approximately 4-5 years of nursing practice/experience; OR a Doctoral degree in nursing or related field and appropriate basic or advanced certification desired, and approximately 3-4 years of nursing practice/experience.

Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements: Lifting up to 35
• pounds for 8-12 hours, intermittently, moderate carrying (15-35 pounds) for 8 to 12 hours, intermittently, lifting over 35 pounds with assistive devices in accordance with the Safe Patient Handling Policy except in emergent situations requiring prompt patient care, pulling for 8 to 12 hours intermittently, pushing for 8-12 hours, intermittently, reaching above the shoulders for 8-12 hours, intermittently, both hands required, walking 8-12 hours, intermittently, standing 8-12 hours intermittently, kneeling 8 to 12 hours, intermittently, repeated bending for 8-12 hours, intermittently, climbing and use of both legs and arms for 8-12 hours, intermittently, both legs required, near vision at 13 to 16" Jaeger 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other eye, ability to distinguish basic colors, hearing aid permitted, twisting, simple grasp and fine manipulation.
Education
The Associate Degree Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science degree in Nursing (MSN) must be fully accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission On Collegiate Nursing Education (CCNE).   • If your school has changed names, or is no longer in existence, you must provide this information in your application.
