Job Description
Responsibilities:
The CSS will work closely with the AOT Co-Occurring Therapist to provide targeted case management services to the individual receiving SUD/COD treatment through the AOT program.
Duties:
- Obtain and maintain proper certification to perform Community Support services, taking full ownership of this process. This includes completing required annual training, maintaining supportive documentation, and submitting applications independently and timely.
- Provide case management services for the clients enrolled in the Assisted Outpatient Program addressing the four domains of recovery: home, health, community, and purpose.
- Provide ongoing assessment of clients’ needs and develop appropriate treatment planning.
- Ensure adherence to the National Standards for Culturally and Linguistically Appropriate Services (CLAS) in Health and Health Care; motivational leadership, proficient in teambuilding, conflict resolution and strength-based management.
- Conduct outreach and engagement activities to reach individuals in Lafayette County appropriate for the AOT program as well as related service providers/community resources.
- Conduct GPRA/NOMS data collection and assist with reporting as required by grant.
- Provide supportive counseling to clients and their families as needed.
- Assist AOT clients in making applications for benefits and assistance.
- Provide assistance to clients regarding housing.
- Provide assistance to clients in securing medications and developing medication management plan.
- Acting as a member of a multi-disciplinary team, monitor participation and progress in treatment programs to assure the planned provision of service according to the individual’s treatment plan and provide referral services for those enrolled in the project when beneficial.
- Accompany the individual to appointments when necessary, this may include extensive travel throughout Region 2.
- Attend Chancery court hearings to discuss clients’ treatment progress with the judge.
- Participate as a member of the AOT expansion team and Steering Committee, and link appropriate family members with the Family Advisory Council.
- Participate in all required training, consultation, and evaluation procedures required by the project, and local/national consultants.
- Assist with the continued growth and development of the steering committee.
- Complete all mandatory documentation to monitor individuals’ progress towards achievement of individualized treatment plan goals, grant related evaluations and associated documents, and complete and submit any requested reports to the Project Director in a timely manner.
- Perform duties related to on-call crisis team as assigned by Mobile Crisis Team Coordinator and Project Director.
- Demonstrate appropriate interactions with coworkers that promote a pleasant work environment.
20. Always maintains client confidentiality.
21. Completes additional job duties assigned by the supervisor.
Qualifications:
- Bachelor’s degree in social work or related field. Certified and credentialed by the State of Mississippi Department of Mental Health.
- 3 years of experience serving SMI adults preferred.
- Ability to relate effectively to severely mentally ill individuals.
- Have transportation, hold a valid driver’s license, have automobile insurance, and have a safe driving record as extensive travel throughout region is required.
