Job Description
Position Summary: Responsible for maintenance and revision of developed systems within the Developmental Disabilities (DD) program including quality assurance systems, training systems, and billing systems. Responsible for keeping these systems organized, efficient and for ensuring that staff is using these systems appropriately. The Program Coordinator also imparts supervises staff within the Day Habilitation program.
Essential Responsibilities:
1. Coordinates delivery of services with Day Habilitation Program Services; parents of those living at home.
2. Assist in the development of individual team productivity, and allocate resources and staff.
3. Monitor and supervise Day Habilitation Client Coordinators and Direct Care Workers.
4. Conduct staff meetings as needed with Field Supervisor, Direct Care staff and Client Coordinators.
5. Review all paperwork prepared by the Client Coordinators on a regular basis.
6. Responsible for evaluating the staff’s performance and writing and communicating performance related issues.
7. Conduct monthly staff meetings with Day Habilitation Staff.
8. Establish procedures and protocols for the integration of special services in the Day Habilitation program.
9. Consult and collaborate on issues impacting the facility.
10. Assess, review and keep records of all consumers.
11. Assess and schedule training for new and old staff.
12. Attend Day Habilitation management meeting and agency wide management meeting.
13. Schedule fire drills and emergency evacuation drills for all all-day habilitation participants, chart their progress and maintain records.
Qualifications: Bachelor's degree in the Human Services field, with one (1) year of related work experience, Associated degree, with (5) years of related work experience or a High School Diploma/GED, with 10 years of work experience with the developmentally disabled community. SCIP, AMAP, and CPR/First Aid certifications preferred, and Familiarity with OPWDD regulations. Must have a valid New York State Drivers License.
