APP – Perioperative / Post Hospital Optimization Clinic
Benefis Health System in Great Falls, Montana is currently recruiting for an advanced practice provider in a new role providing perioperative and post hospital clinical care.
Position details:
- Ideal candidate will have experience as an advanced practice provider in a primary or specialty area and a strong desire to learn and grow with ability to see the span of all age groups.
- Participate in a multidisciplinary, multimodal approach based on the patient’s needs, beginning with an assessment of patient and reviewing their labs.
- Help assess the medical conditions of high-risk patients and provide information to anesthesiologists and surgeons to optimize patient status after surgery, reduce last-minute cancellations.
- Provide interventions prior to surgery to allow for improved function & improvement in physical health before major surgery.
- Provide post hospital follow up within 7-14 days of discharge for those patients that cannot get into their primary care provider to touch base on medication management, physical evaluation and help with coordination of care, if need be, to help reduce the readmission rate of patients.
Work hours and call coverage:
- General clinic hours will be M – F from 7:00 am – 4:00 pm
- Care will be provided to patients over the phone, via telemed, and occasionally in person.
