Connected-Mid Valley Home Health & Hospice is hiring a After HoursLicensed Practical Nurse (LPN) to serve in our new Willamette Valley area location (Salem/Woodburn). The After Hours LPN is responsible for following the patient's plan of care and providing treatment under the supervisor of the RN nights and/or weekends.

The After Hours LPN is responsible for following the patient's plan of care and providing treatment under the supervisor of the RN nights and/or weekends.
Essential Duties
- Implement plan of care initiated by the registered nurse
- Provide accurate and timely documentation consistent with the plan of care
- Collect assessment data and performs observation to report to and collaborate with RN(s) and physician(s)
- Participate in coordination of home health services with interdisciplinary team
- Use equipment and supplies effectively and efficiently
- Participate in personal and professional growth and development
Requirements and Qualifications
- Valid Oregon Licensed Practical Nurse License
- Maintain a current CPR certification
- 2+ years of experience working in direct patient care
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
