Description
The Hennepin County Public Health Clinic (PHC) is seeking an African American HIV Prevention Specialist to work in the African American community and support our HIV outreach and prevention efforts, especially among African American populations most affected by HIV contraction. Interested individuals, BIPOC community members, and individuals with related experience are encouraged to apply. This position will increase the awareness, uptake, and coordinated usage of Red Door Clinics' services among African Americans by providing sexual education, outreach, and prevention services within a multidisciplinary team of non-medical and medical staff that includes nurse practitioners, registered nurses, medical assistants, and community health specialists. This position will also provide forward facing, community engagement to support Red Door Clinic's focus on increasing access to PrEP, HIV testing, and other sexual health services. Our clinical services are tailored to serve communities disproportionately impacted by HIV, STIs, and other related disparities.

Location and hours:
This position is hybrid and will be performed both on-site at the Public Health Clinic located at 525 Portland Avenue, Minneapolis, MN 55415 and remote as job duties require. Currently, the clinic is open Monday, Tuesday, Wednesday, and Friday 8 a.m. to 4 p.m., and Thursday 10 a.m. to 4 p.m. This position will require working some evening hours when the clinic resumes evening hours on Mondays and Wednesdays until 7:30 p.m. No regular weekend, overnight or holiday shifts are required. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.
New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will:
  • Provide individual, client-centered services to Red Door clients that include HIV and STI education, client intake and service initiation, risk reduction, and counseling.
  • Identify, seek out, and engage with locations and organizations throughout Hennepin County that will support increased engagement with African American communities, especially gay, bisexual, MSM, transgender, and queer subpopulations.
  • Work with an interdisciplinary team within the clinic and community that includes outreach workers, nurse practitioners, public health nurses, medical assistants, clinical support staff, and community organizations to support client needs.
  • Coordinate and collaborate with other Public Health Clinic prevention programs and services, including HIV care linkage services, non-medical case manager, benefits navigation, harm reduction counseling, syringe services, MOUD, PrEP, and reproductive health education when indicated, to better serve communities and populations disproportionately affected by sexual health disparities.
  • Provide health education and connection to resources to African American gay, bisexual, and MSM through education, discussions, presentations, and other effective modalities of engagement.
  • Assist individuals to gain a better understanding of their behaviors and how they contribute to increased likelihood of HIV acquisition, provide HIV testing, counseling, PrEP, linkage to care, syringe exchange, sexual and public health services, and external resources to support client needs.
  • Serve as a liaison of the Red Door Clinic when interfacing with African Americans, HIV service organizations, community groups and organizations, health care professionals, and other external partners.

Need to have:
  • One of the following:
    • Bachelors degree in public health, nursing, behavioral/social science, or an approved related field.
    • Four years or more of related professional experience providing HIV testing, health education, counseling, outreach, engagement, and referral services with communities disproportionately affected by HIV and STIs.

Nice to have:
  • Experience:
    • Working with and supporting BIPOC (e.g., Native American, Latinx, and African American), transgender, gender non-conforming, and LGBTQ+ communities.
    • Collecting, tracking, and reporting program data.
    • Working with EPIC and/or electronic medical records (EMR).
  • Knowledge of:
    • Community resources and the ability to conduct motivational interview to assess behaviors and need for services.
    • HIV harm reduction assessments to identify persons who may benefit from PrEP, sexual health education, non-medical case management, external referrals, and other interventions.
    • Current public health practices/principles and education/training methods.
    • Basic skills in Microsoft Office (Word, Excel, PowerPoint, and Outlook Teams) and other electronic emails, keyboarding, and the use of internet and intranet.
  • Ability to:
    • Embrace the value of different lived experiences across diverse communities.
    • Exercise independent judgment and initiative in performing duties.
    • Communicate effectively, both verbally and in writing.
    • Conduct prevention services through data informed and equitable lenses.
