Health At Home Consultants is looking for an APRN with immediate availability for part time or full time in the Lincoln/Omaha/and surrounding areas to assist our practice. Our providers travel daily to Assisted Living Communities, Skilled Nursing Facilities, Memory Support Communities, and Independent Living Facilities and residential homes. We are designed to meet the needs of adult/geriatric patients that are unable to get out to their PCP’s office. We treat acute needs as well as chronic conditions either as the patient’s primary provider or as supplemental care. One of our nurses will be traveling alongside you through your orientation to assist with patient care that has experience in each of these communities and with the patients. Health at Home Consultants is looking for an APRN that is experienced in acute care. Your main focus would be in skilled communities with some assisted living.
