POSITION SUMMARY
The APN supports physicians and their patients by providing advanced nursing care to patients presenting with uncomplicated/acute clinical problems. Provides operational assistance to assure quality of care and service in a cost effective manner which is consistent with the applicable state standards for licensure for the position and the organizational practice guidelines.
ESSENTIAL DUTIES AND RESPONSIBLITIES (Included but not limited to the following);
· Elicits histories and perform physical examinations on patients presenting with routine uncomplicated and acute medical/surgical conditions to establish baseline health profile.
· Performs and/or assists in the administration of routine diagnostic/laboratory studies, clinical and therapeutic procedures.
· Develops presumptive diagnoses and implements treatment plans based on advanced nursing assessment/problem identification consistent with established practice guidelines.
· Documents assessments of patients’ health and illness status.
· Evaluates patient responses to treatment plans and redirects patient care based on findings.
· Assesses patient clinical/health education needs. Instructs/counsels patients, in one-on-one and small group settings, regarding health matters (e.g. diets, disease therapy, normal growth and development, etc.).
· Refers patients to clinical/health education classes, as appropriate for more in-depth services.
· Provides telephone care to assigned patients, as necessary.
· Provides dressing changes, etc. as necessary.
· Obtains assigned patient vital signs and provide documentation.
· Provides on site patient care and outreach for our School Based Health Clinic and/or Mobile Health Unit.
§ Acts as a resource person to other staff members.
QUALIFICATIONS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
EDUCATION AND EXPERIENCE
· Completion of an accredited Master’s Degree program for Family Nurse Practitioners
· Experience based on current needs of the practice at the time of hiring.
CERTIFICATES AND LICENSES
· Current Illinois RN
· Full Practice Authority APRN License
· CPR Certification

Ability to read, comprehend and communicate detailed instructions. Ability to prepare correspondence and memos. Ability to effectively present information in one-on-one and small group situations to patients, physicians and other employees of the organization.

WORKING CONDITIONS
Clinical Setting.
OSHA – Highly likely to produce blood or body fluid exposure.
Flexibility to work at various clinic sites as needed.
Some weekend hours required.
Occasional extended clinic hours required.

Pay: $95,000.00 - $105,000.00 per year
