Principal Duties & Responsibilities:
- Complete intake assessments and follow-ups for medication management.
- Provides patient assessment, diagnosis, and treatment plans in accordance with statutes, regulations and protocols regulating the profession.
- Collaborates with treatment team(s).
- Provides psychiatric health services, education, counseling, and emotional support.
- Refers patients for inpatient care, in collaboration with the Medical Director.
- Issues medication prescriptions in accordance with treatment guidelines.
- Orders laboratory tests, interprets, and explains the test results to patients.
- Provides patient and treatment team education regarding medications, risks, benefits, and reasonable outcome expectations.
- Communicates with patients and treatment teams and engages in follow-up as necessary.
- Provide patient education regarding medications; laboratory procedures and results; diet and exercise, sleep hygiene, mindfulness, and other lifestyle practices, community and online resources (e.g local or online support groups or information sites for patients and families).
- Perform medication reconciliation between pharmacy, electronic health record, and group home medication packets.
- Oversee collection of clinical data in group home settings, including sleep logs, elimination functions, “ins and outs” for fluid intake management, use of PRN medications. Train and/or collaborate with group home staff as needed.
- Provide leadership and expertise to group home staff in caring for residents with chronic medical conditions, e.g diabetes, asthma, arthritis or other musculoskeletal disorders. Work within a treatment team to help provide coordinated mental/behavioral health care to patients.
Required Skills, Abilities & Education:
- MSN or DNP (or equivalent) APRN, PMHNP board-certified or eligible with 1+ year psychiatric nursing experience
- Sound understanding of all federal and state regulations including HIPAA and OSHA
